针刺夹脊穴及督脉穴对急性脊髓损伤神经传导通路影响的研究 被引量:29

摘要 目的探讨针刺夹脊穴及督脉穴对急性脊髓损伤神经传导通路的影响和作用机制。方法取健康清洁级新西兰兔80只,将其按随机数字表法随机分为5组,分别为正常对照组、模型对照组、假手术组、针刺穴位组、甲基强的松龙(MP)疗法组,每组16只,造模成功后进行MR检测鉴定。正常对照组、模型对照组、假手术组不予任何治疗。针刺组、MP冲击疗法组采用改良Allens法制作脊髓中度损伤模型后,针刺组给予夹脊穴及督脉穴电针治疗,采用连续脉冲,频率2 Hz,输出电流0.6~1 m A,每次30 min,每日1次,共7次;MP疗法组造模后即按30 mg/kg首剂静注1次,1 h后即开始应用按5.4 mg/kg计算23 h总量,分4次静脉注射。1周后对各组兔子进行一般行为,神经功能,损伤截段HE染色,辣根过氧化物酶(HRP)逆行示踪评价和对比。结果在精神状况、反应能力、活动度方面,术后7 d时正常对照组、假手术组动物均基本恢复至术前水平,针刺穴位组较MP疗法组、模型对照组恢复状况要好,差异有统计学意义(P〈0.05)。正常对照组Reuter评分无变化,假手术组术后7 d恢复到术前水平;模型对照组、MP疗法组、针刺穴位组术后7 d Reuter评分均降低,针刺穴位组与模型对照组、MP疗法组比较降低更加明显,差异有统计学意义(P〈0.05)。急性脊髓损伤7 d后,HRP呈蓝黑色表达于脊髓灰质中,相较于模型组,针刺组与MP组明显使受损脊髓组织表达更多,但可能是时间原因,仍无法达到假手术组水平,提示针灸对脊髓神经通路的再通是有一定作用的;与此同时,针灸组与MP组比较,无明显差异。而HRP阳性表达强度的光密度值与HE染色结果吻合。结论针刺能明显改善急性脊髓损伤的神经和运动功能,其机制可能为调节红核神经元细胞,使得损伤截段神经通路修复而产生作用。 Objection:To explore the effects and mechanism of needling Hua Tuo Jiaji points and Du meridian points on acute spinal cord injury(sci)nerve conduction pathway.Methods:80 health clean New Zealand rabbits were randomly divided into 5 groups according to random number table method:the normal control group,the model control group,sham operation group,acupuncture group and MP shock therapy group,16 cases in each.After establishing models,MR detection identification was taken.The normal control group,the model control group and sham operation group will not take any treatment.The spinal cord injury model was made with modified Allens method in MP shock therapy group,while the acupuncture group was treated by Electroacupuncturing Hua Tuo Jiaji points and Du meridian acupuncture points,with continuous pulse,frequency of 2 hz and output current0.6~1 m A for 30 min,once a day for seven days.MP shock therapy group took 30 mg/kg first agent static note 1time after established models,1 h after the start application according to 5.4 mg/kg to calculate total 23 h,intravenous four times.1 week later,the rabbit general behavior,nerve function and HE staining of damage the cut section,HRP retrograde tracer of each groups were evaluated and compared.Results:1 week later,general behavior of each group was improved.For neural function,Acupuncture and MP groups were obviously better than the control group(the difference was statistically significant,P〈0.01).HE staining showed hemorrhage,edema and necrosis area of the acupuncture group were obviously better than those of the model control group and neurons and glial cells were improved.HRP retrograde tracer in the acupuncture group and MP group was better than that of the model group,and absorbance difference was statistically significant compared with the model group(P〈0.05).Conclusion:Acupuncture can obviously improve the acute spinal cord injury of nerve and motor function,whose mechanism may be to adjust the red nucleus neurons and finnaly make the dama
