动脉导管未闭介入封堵术前后肺动脉压及血压改变的对比研究

摘要 目的分析应用国产封堵器介入封堵动脉导管未闭(PDA)术前后血流动力学的变化规律。方法选择2014年6月至2015年2月在我科住院的PDA患者,应用国产封堵器进行介入封堵治疗,分别于封堵术前及封堵术后测主动脉压及肺动脉压进行对比分析。结果 64例PDA患者均经导管封堵成功(技术成功率100%)。与封堵术前比较,术后肺动脉收缩压[PASP,(58±24)mmHg VS(36±11)mmHg]、肺动脉舒张压[PADP,(27±15)mmHg VS(14±6)mmHg]、平均肺动脉压[MPAP,(37±18)mmHg VS(22±6)mmHg]均显著降低(P均<0.01)。但封堵术后主动脉压升高,收缩压由(111±25)mmHg升至(126±32)mmHg(P<0.05)。结论经导管封堵PDA操作简便、技术成功率高、疗效可靠。术后肺动脉压明显下降。 Objective To analyze the hemodynamic change rules in transcatheter closure of patent ductus arteriosus(PDA).Methods 64 patients with PDA underwent transcatheter closure by using domestic occlude from Jun2014 to Feb 2015.Aortic pressure and pulmonary arterial pressure were measured during and after the operation,respectively.Results The success rate of transcatheter closure was 100%.Compared with before the closure,there were significant decrease in pulmonary arterial systolic pressure[PASP,(58±24)mmHg vs.(36±11)mmHg],pulmonary arterial diastolic pressure[PADP,(27±15)mmHg vs.(14±6)mmHg]and mean pulmonary arterial pressure[MPAP,(37±18)mmHg vs.(22±6)mmHg].But aortic systolic pressure rose from(111±25)mmHg to(126±32)mmHg after transcatheter closure.Conclusion Transcatheter closure of PDA with occlude is a simple and efficient method with a high success rate and good occlusion effects.Pulmonary arterial pressure is significantly decreased after successful intervention.
