应用XpertMTB/RIF技术快速诊断膝关节结核 被引量:10

摘要 目的评价利福平耐药实时荧光定量核酸扩增检测技术(XpertMTB/RIF,简称“Xpert”)在膝关节结核诊断中的临床应用价值。方法连续收集2014年1月至2015年10月在首都医科大学附属北京胸科医院骨科接受手术治疗的49例膝关节病变患者的关节积液标本,每份标本同时分别行涂片抗酸杆菌检查、罗氏培养基培养、Xpert检测。以临床诊断为参考标准,评价Xpert检测关节积液标本中结核分枝杆菌的敏感度和特异度。结果以临床诊断为参考标准,涂片、罗氏培养基培养、Xpert检测的敏感度分别为17.2%(5/29)、31.0%(9/29)、69.0%(20/29),差异有统计学意义(X2=8.34,P%0.05);特异度分别为85.0%(17/20)、100.0%(20/20)、90.O%(18/20)。结论Xpert检测关节积液标本中结核分枝杆菌的敏感度高,并且可同时检测其耐药性,可用于快速诊断膝关节结核。 Objective To evaluate the clinical value of Xpert MTB/RIF (Xpert) for the rapid diagnosis of knee joint tuberculosis. Methods Forty-nine joint effusion specimens from patients with knee disease admitted in Department of Orthopedic, Beijing Chest Hospital, Capital Medical University were collected from Jan. 2014 to Oct. 2015. Acid fast bacteria smear, Lowenstein-Jensen culture and Xpert detection were performed for each specimen. The sensitivity and specificity of Xpert were evaluated according to clinical diagnosis as reference standard. Results According to the clinical diagnosis as reference standard, the sensitivity of acid fast bacteria smear, Lowenstein-Jensen culture and Xpert were 17.2% (5/29),31.0% (9/29) and 69.0% (20/29), and the difference was significant (Z2 = 8.34, P 〈 0.05) ;the specificity were 85.0% (17/20), 100.0% (20/20), and 90.0 (18/20), respectively. Conclusion Xpert has high sensitivity in diagnosis of joint effusion infected by Mtb and detects simultaneously the drug resistance. It can be used to diagnose rapidly for knee joint tuberculosis.
