摘要
目的探讨无精子症患者雌二醇(E_2)与雌激素受体(ER)α、ERβ间的相关性及其对精子的影响。方法选择45例无精子症患者,检测其外周血中的促卵泡激素(FSH)、促黄体生成素(LH)、E_2、ERα、ERβ水平;行睾丸穿刺,观察取得的生精小管中有无精子,检测上清液中的E_2、ERα、ERβ水平。根据睾丸穿刺结果和FSH水平,将患者分为有精子低FSH组(睾丸穿刺有精子,FSH<8U/L)、无精子低FSH组(睾丸穿刺无精子,FSH<8U/L)、无精子高FSH组(睾丸穿刺无精子,FSH>8 U/L),每组15例。比较3组患者外周血FSH、LH水平,以及外周血与睾丸组织中的E_2、ERα、ERβ水平,分析FSH、LH、E_2、ERα、ERβ间的直线相关关系。结果 3组间患者外周血中的E_2、ERα、ERβ和睾丸组织中的E_2水平的差异均有统计学意义(P值均<0.05);有精子低FSH组、无精子低FSH组、无精子高FSH组患者外周血中ERα、ERβ水平和睾丸组织中的E_2水平均逐渐升高(P值均<0.05),外周血中的E_2水平逐渐降低(P值均<0.05)。3组间睾丸体积和睾丸组织中的ERα、ERβ水平的差异均无统计学意义(P值均>0.05)。外周血中的FSH水平与外周血中的E_2水平呈负相关(r=-0.522,P=0.026),与外周血中的ERα水平(r=0.766,P=0.001)、外周血中的ERβ水平(r=0.723,P=0.001)和睾丸组织中的E_2水平(r=0.597,P=0.019)呈正相关;外周血中的LH水平与睾丸组织中的E_2水平(r=0.610,P=0.016)呈正相关;外周血中的E_2水平与外周血中的ERα水平(r=-0.777,P=0.001)、外周血中的ERβ水平(r=-0.709,P=0.003)和睾丸组织中的E_2水平(r=-0.537,P=0.018)呈负相关;睾丸组织中的E_2水平与睾丸组织中的ERα水平(r=0.681,P=0.004)、睾丸组织中的ERβ水平(r=0.802,P=0.000)呈正相关。结论睾丸体积正常的无精子症患者,随着睾丸生精功能的降低,外周血中的E_2水平降低,而外周血中的ERα和ERβ水平升高,正/负反馈作用于促性腺激素FSH。随着FSH水平的升高,正反馈调节睾
Objective To explore the correlation between 1 estradiol(E_2)and estrogen receptors(ER)α,ERβand their impacts for spermatogenesis in patients with azoospermia.Methods A total of 45 patients with azoospermia were enrolled in this study.Venous blood was collected and the levels of follicle-stimulating hormone(FSH),luteinizing hormone(LH),E_2,ERαand ERβwere measured.Then testicles were punctured to judge whether there were sperms or not.And the levels of E_2,ERαand ERβin supernatant were detected.According the results of puncture and FSH concentration,the patients were divided into three groups:group A(there were sperms in testicles,FSH8U/L),group B(no sperm,FSH8U/L)and group C(no sperm,FSH8U/L).There were 15 patients in each group.The levels of FSH,LH,E_2,ERαand ERβwere compared between groups.The correlation of FSH,LH,E_2,ERαand ERβwere analyzed by linear analysis.Results There were significant differences in E_2,ERαand ERβof venous blood and E_2 concentration of testicular tissue between groups(all P〈0.05).The levels of ERαand ERβof venous blood and E_2 concentration of testicular tissue were gradually elevated in group A,B and C orderly(all P〈0.05),while the level of E_2 of venous blood were gradually decreased.There were no significant differences in terms of testicular volume,or the levels of ERα,ERβof testicular tissue between groups(all P〉0.05).The concentration of FSH of venous blood was positively correlated with ERαof venous blood(r=0.766,P=0.001),ERβ(r=0.723,P=0.001)of venous blood,and E_2 of testicular tissue(r=0.597,P=0.019),while negatively correlated with E_2 of testicular tissue(r=-0.522,P=0.026).The concentration of LH of venous blood was positively correlated with E_2 of testicular tissue(r=0.610,P=0.016).The level of E_2 was negatively correlated with ERαin venous blood(r=-0.777,P=0.001).The level of ERβin venous blood(r=-0.709,P=0.003)was negatively correlated with E_2 of testicular tissue(r=-0.537,
