摘要 目的超声造影是一种无创性检查技术,本研究分析常规超声及超声造影测量门静脉高压症脾脏和肝脏的参数变化,探讨超声造影在门静脉高压症中的应用价值。资料与方法选取经临床证实为门静脉高压症的37例患者作为观察组,同期选取29例非门静脉高压症患者作为对照组,采用常规超声测量两组脾脏大小、门静脉内径(PVD)、门静脉血流速度(PVV)、肝动脉阻力指数(HARI)、脾静脉内径(SVD)、脾静脉血流速度(SVV),采用造影剂增强超声测量肝动脉开始显影时间(HAAT)、门静脉开始显影时间(PVAT)及肝静脉开始显影时间(HVAT),计算两组肝动静脉渡越时间(HA-VTT)、门静脉肝静脉渡越时间(PV-HVTT)及肝动脉门静脉渡过时间(HA-PVTT)。比较两组超声及超声造影各参数的差异。结果观察组较对照组PVD、SVD、HARI、脾脏厚度、脾脏长径明显增大,PVV、SVV明显减少(P<0.05)。Child-Pugh B级门静脉主干内径与A级比较差异有统计学意义(P<0.05)。观察组较对照组HVAT、HA-VTT及PV-HVTT明显缩短,HA-PVTT明显延长(P<0.05)。结论 HVAT、HA-VTT、PV-HVTT及HA-PVTT可以准确地反映门静脉高压症肝内血流灌注情况,超声造影可以更好地评估门静脉高压症。 Purpose Contrast-enhanced ultrasonography is a noninvasive technique. This study aims to explore the significant of conventional ultrasound(US) combined with contrast-enhanced ultrasound(CEUS) in diagnosis of portal hypertension(PH). Materials and Methods Thirty-seven patients with PH confirmed in clinic were recruited in the study. Twenty-nine non-PH patients were included as a control group. All the subjects underwent conventional US and CEUS examinations. The parameters including spleen size, portal vein diameter(PVD), portal vein flow velocity(PVV), hepatic artery resistance index(HARI), splenic vein diameter(SVD), splenic vein flow velocity(SVV), hepatic artery arriving time(HAAT), portal vein arriving time(PVAT) and hepatic vein arriving time(HVAT) were compared between the two groups. HAVTT, PV-HVTT and HA-PVTT were calculated. Statistical difference of the parameters between the two groups was analyzed. Results PVD, SVD, HARI, spleen thickness and spleen size were significant higher in PH group than those in control group, while PVV and SVV were significant lower(P〈0.05). Portal vein's diameter of PH group were larger with liver function grade A than with grade B(P〈0.05). HVAT, HAVTT, PV-HVTT were significant shorter in PH group than in control group, while HA-PVTT were significant longer(P〈0.05). There were no statistical significance in HAAT and PVAT difference between the two groups(P〈0.05). Conclusion HVAT, HAVTT, PV-HVTT and HA-PVTT could reflect the intrahepatic blood perfusion changes in patients with portal hypertension. Conventional ultrasound combined with contrast-enhanced ultrasonography may evaluate portal hypertension better.
