摘要
目的:探讨血清降钙素原(PCT)、B型钠尿肽(BNP)联合检测对脓毒症患者的早期诊断及预后评估价值。方法:110例脓毒症患者分为存活组(n=67)和死亡组(n=43)。分别于入院时及入院24h取静脉血行PCT及BNP测定,并比较患者24h内急性生理学与慢性健康状况评分(APACHE–II)。结果:2组入院时PCT、BNP及APACHE II评分比较差异无统计学意义(P>0.05)。入院24h,存活组PCT、BNP及APACHE II评分均下降,死亡组均上升,与入院时比较差异具有统计学意义(P<0.05);存活组PCT、BNP及APACHE II评分均低于死亡组,2组比较差异具有统计学意义(P<0.05)。Pearson相关性分析表明,PCT(r=0.521,P=0.013)、BNP(r=0.452,P=0.029),均与APACHE II评分呈统计学相关。PCT及BNP联合检测对患者死亡预测的敏感性为89.4%,特异性为91.6%。结论:PCT及BNP联合可提高脓毒症检测的敏感性和特异性,PCT及BNP持续上升可提示患者预后不良。
Objective.. To investigate the detection and prognosis value of procalaitonin(PCT) and B natri uretic peptide(BNP) in patients with sepsis. Methods~ One hundred and ten patients with sepsis in the department of ICU in our hospital from January 2014 to June 2015 were divided into survival group (n=67) and death group (n =43) , according to the treatment prognosis. The PCT and BNP from veinal blood at admission and 24 h after admission were measured. And the acute physiology and chronic health status score (APACHE- II) in the two groups were also compared. Results.. There was no significant difference of the APACHE-II scores, BNP and PCT between the two groups at admission (P〉0.05). 24 h after admission, the APACHE-II scores, BNP and PCT in the surviv al group were all decreased, while increased in the death group, and there were all statistical significance difference compared with that at admission(P〈0.05). And the APACHE-II scores, BNP and PCT in the survival group were less than those in the death group(P〈0.05). Pearson correlation analysis showed that PCT (r=0. 521 ,P=0. 013) and BNP (r=0. 452, P=0. 029) both had statistically significant with APACHE-II scores. The sensitivity and specificity of PCT and BNP combined detection to the mortality of patients was 89.4M and 91.6%0. Conclusion.. The combination of PCT and BNP can improve the sensitivity and specificity of detection of sepsis, and the sustained rise of which may be associated with the poor prognosis.
