α硫辛酸联合单唾液酸四己糖神经节苷脂治疗糖尿病周围神经病变患者的效果及对相关神经传导速度的影响 被引量:8

The therapeutic effect and effects on nerve conduction velocity of α-lipoic acid combined with monosialoteterahexosyl ganglioside sodium on diabetic peripheral neuropathy

摘要 目的观察α硫辛酸联合单唾液酸四己糖神经节苷脂(以下简称神经节苷脂)治疗糖尿病周围神经病变(DPN)患者的效果及对相关神经传导速度的影响。方法将纳入研究的96例DPN患者随机分为观察组50例和对照组46例。对照组在常规治疗基础上每日给予神经节苷脂40mg静脉滴注;观察组在对照组的基础上每日给予α-硫辛酸600mg静脉滴注。两组疗程均为2周。治疗2周后观察疗效,并行肌电图检查测定正中神经、腓总神经的运动神经传导速度(MNCV)和感觉神经传导速度(SNCV)。结果观察组总有效率94.00%(47/50)高于对照组71.74%(33/46),差异有统计学意义(χ2=7.021,P〈0.05);治疗后,观察组与对照组患者正中神经的MNCV[(47.68±3.34)、(43.82±3.10)m/s]、SNCV[(40.23±2.76)、(37.31±2.79)m/s]与腓总神经的MNCV[(42.86±3.16)、(39.47±2.17)m/s]、SNCV[(35.67±2.61)、(32.16±2.50)m/s]均较治疗前改善,与治疗前比较差异均有统计学意义(观察组治疗前后比较,t值分别为7.876、8.428、6.923、7.234;对照组治疗前后比较,t值分别为3.572、3.483、3.413、4.182,P均〈0.05),但观察组改善更明显,与对照组治疗后比较差异有统计学意义(t值分别为5.133、3.827、4.634、4.546,P均〈0.05)。结论仅.硫辛酸联合神经节苷脂治疗DPN可显著改善患者临床症状及提高相关神经传导速度,优于单用神经节苷脂。 Objective To observe the therapeutic effect and effects on nerve conduction velocity of The therapeutic effect and effects on nerve conduction velocity of α-lipoic acid combined with monosialoteterahexosyl ganglioside sodium on diabetic peripheral neuropathy- lipoic acid combined with monosialoteterahexosyl ganglioside sodium( GM1 ) on diabetic peripheral neuropathy (DPN). Methods Ninety-six cases patients with DPN were randomly divided into the observation group (50 cases ) and the control group (46 cases). The control group received GM 1 40 mg daily though travenous drip, the observation group received The therapeutic effect and effects on nerve conduction velocity of α-lipoic acid combined with monosialoteterahexosyl ganglioside sodium on diabetic peripheral neuropathy-lipoic acid 600 mg and daily though travenous drip. The course of treatment was 2 weeks in two groups. The effect was observed after 2 weeks, the motor nerve conduction velocity ( MNCV ) and sensory nerve conduction velocity ( SNCV ) of median nerve and common peroneal nerve were detected by EMG. Results The total effectiveness of observation group was 94.00% (47/50) ,higher than that of the control group(71.74%( 33/46), χ2= 7. 0205, P〈0. 05 ). After the treatment, MNCV and SNCV of median nerve of observation group and the control group were (47. 68±3.34) m/s and (43.82±3.10)m/s, (40. 23±2. 76)m/s, ( 37. 31 ± 2. 79) m/s, and MNCV and SNCV of common peroneal nerve of observation group and the control group were (42. 86± 3.16) rn/s and ( 39.47 ±2. 17 ) m/s, ( 35.67 ± 2. 61 ) rn/s and ( 32. 16 ±2. 50 ) m/s, all improved compared to before treatment( observation grou : t = 7. 876,8. 428,6. 923,7. 234; control grou : t = 3. 572,3. 483, 3.413,4. 182;P〈0. 05 ) , and the magnitude of improvement of the observation group was more significant, and the difference was significant compared to control group after six months (t = 5. 133,3. 827,4. 634,4. 545; P 〈0. 05
