胫后动脉穿支逆行岛状皮瓣修复小腿中下段胫前创面 被引量:1

摘要 目的探讨应用胫后动脉穿支逆行岛状皮瓣修复小腿中下段胫前创面的临床效果。方法对2013年5月-2014年5月收治的10例小腿中下段胫前创面采用胫后动脉穿支逆行岛状皮瓣修复,供区游离植皮。结果术后10例皮瓣全部顺利成活,小腿供区创面植皮后全部成活,随访2-12个月,皮瓣血运良好.色泽接近周围正常皮肤,外形无臃肿,厚薄适中,有部分感觉恢复。结论胫后动脉穿支逆行岛状皮瓣不牺牲胫后动脉主干、小腿深筋膜、大隐静脉和隐神经,以内踝尖上4.0~10.0cm节段内穿支为蒂.可以修复小腿中下段胫前创面,适应证广,血管表浅恒定,手术操作简单,手术时间短,成活率高,外形佳,值得推广。 Objective To explore the clinical application and effect of repairing the pretibial soft-tissue defects in distal lower legs with the reversed island flap pedicled with the posterior tibial artery. Methods To repair 10 cases of the pretibial soft-tissue defects in distal lower legs with the reversed island flap pedicled with the posterior tibial artery, and to do free skin-grafting in the donor sites. Results All 10 cases of skin flap survived. The skin in the skin-grafted sites of distal lower legs all survived, the follow-up time was 2 to 12 months. The skin flaps had good blood supply, and the color of skin flaps resembled that of the surrounding skin. The skin flaps were not bulky, And the appearances were satisfactory. The thickness is moderate, the senses of some skin flaps returned. Conclusion The blood supply of the posterior tibial artery is constant and reliable. The surgery will not sacrifice the main force of posterior tibial artery, or the great saphenous veins and saphenous nerves. The surgery used the island shape skin flap pedicled with 4.0~10.0 cm of inner perforating branch on the pinnacle of malleolus medialis to repair the pretibial soft-tissue defects in distal lower legs. The blood vessel is shallow and constant, and the operation of is simple and fast. The survival rate for the skin flaps is high, and the appearances of the skin flaps are fine, hence this method is worth popularized.
