摘要
目的探讨远端胃癌D2根治术中影响手术的腹腔干变异及其意义。方法回顾性分析2013年2月~2015年2月我科收治的163例胃癌D2根治术中发现的影响手术的腹腔干变异情况。结果 163例中发现影响手术的腹腔干变异4例,1例肝总动脉起源于胃左动脉,1例胃左动脉缺失由肝总动脉发出3只胃分支动脉替代,2例胃左动脉起独立源于腹主动脉。结论远端胃癌根治术时应考虑到腹腔干血管变异的可能,小心解剖变异血管,避免损伤脏器的血供。
Objective To explore the surgical value of celiac axis variations in the D2 lymph node dissection for distal gastric cancer. Method The celiac axis variations were analyzed retrospectively in the 163 gastric cancer patients performed D2 lymph node dissection from February 2013 to February 2015. Result There were celiac axis variations in 4 cases,including 1 case of common hepatic artery originated from left gastric artery,1 case of 3 gastric artery branches originated from common hepatic artery without left gastric artery,and 2 cases of left gastric artery originated from abdominal aorta. Conclusions The variations of celiac axis should be think over during the D2 lymph node dissection for distal gastric cancer. The dissection of vessel variations would be serious to protect the blood supply of visceral organ.
