良、恶性孤立性肺结节采用多层CT灌注成像技术鉴别的诊断比较 被引量:3

Diagnostic Comparison of Multi-Slicel CT Perfusion Imaging Technique in the Identification of Benign and Malignant Solitary Pulmonary Nodules

摘要 目的研究良、恶性孤立性肺结节(SPN)经多层CT灌注成像技术鉴别的诊断差异。方法整群选取该院2013年2月—2014年9月收治的86例SPN为研究对象,其中经临床确诊恶性结节48例记为恶性结节组,良性结节38例记为良性结节组。对该86例SPN患者行多层CT灌注成像技术鉴别诊断,观察两组血容量(BV)、血流量(BF)、平均通过时间(MTT),并在时间—密度曲线(TDC)基础上测量结节—主动脉强化值比(S/A)、结节强化值(HU)、达峰时间(TTP)等指标。比较两组诊断结果差异。结果良性结节组BV(3.92±1.52)m L/100 g、BF(68.57±12.31)m L/min/100 g较恶性结节组显著较低,差异有统计学意义(P<0.05);良性结节组强化值(8.62±2.41)HU、S/A(0.17±0.04)%较恶性结节组(38.56±10.42)HU、(0.34±0.05)%明显较低,TTP(34.8±5.24)s较恶性结节组(21.4±3.71)s显著较长,差异有统计学意义(P<0.05)。结论良、恶性结节的血流动力和形态学特征采用多层CT灌注成像技术诊断存在显著差异,可作为临床鉴别良、恶性结节的重要依据。 Objective To research the diagnostic difference of multi-Slicel CT perfusion imaging technique in the identification of benign and malignant solitary pulmonary nodules. Methods 86 cases of SPN patients treated in our hospital from February 2013 to September 2014 were extracted as the research object and diagnosed as the malignant nodule group with 48 cases and benign nodule group with 38 cases, and all patients were identified and diagnosed by multi-slice CT perfusion imaging technique, the blood volume(BV), blood flow(BF)and mean transit time(MTT)of the two groups were observed, and the indicators of nodule-aorta enhance value ratio(S/A), nodule enhance value(HU)and time to peak(TTP)on the basis of the time-density curve(TDC)were measured, the difference in the diagnostic result between the two groups was compared. Results The BV and BF in the benign nodule group were respectively(3.92±1.52)m L/100 g and(68.57±12.31)m L/min/100 g, which were obviously lower than those in the malignant nodule group, and the difference was statistically significant(P 0.05); the HU and S/A in the benign nodule group were obviously lower than those in the malignant nodule group,(8.62±2.41)vs(38.56±10.42),(0.17±0.04)% vs(0.34±0.05)%, the TTP in the benign nodule group was obviously longer than that in the malignant group(34.8 ±5.24)s vs(21.4 ±3.71)s, and all differences were statistically significant(P〈0.05). Conclusion The application of multi-slice CT perfusion imaging technique in diagnosis of the blood flow dynamics and morphological features of benign and malignant nodules has an obvious difference, which can be used as an important basis for identifying the benign and malignant nodules in clinic.
