摘要
目的探讨囊胚培养及移植在体外受精周期中的应用。方法整群选取2012年1月—2014年12月期间来该院接受体外受精-胚胎移植的64例患者作为该次研究对象,按照选择胚胎培养及移植时期的不同将64例体外受精-胚胎移植患者分成研究组和对照组,研究组为选择囊胚期胚胎培养及移植的患者,共33例,对照组为选择卵裂期胚胎培养及移植的患者,共31例。受精成功后采用G1进行早期胚胎培养,采用G2进行囊胚培养。结果两组患者中各有1例消移植周期病例,实际移植周期数分别为32个和30个,研究组患者的胚胎植入率为37.1%,妊娠率为53.1%,与对照组患者的19.4%和33.3%比较,差异有统计学意义(P<0.05)。两组患者的流产率和异位妊娠率比较差异无统计学意义(P>0.05),但对照组患者的多胎率明显高于研究组患者(P<0.05)。结论选择囊胚期胚胎培养及移植可有效提高胚胎植入率和妊娠率,降低不良妊娠结局发生率和移植周期取消率。
Objective To study the application of culture and transplanting in blastocyst stage in the in vitro fertilization cycle. Methods 64 cases of patients receiving in vitro fertilization-embryo transfer in our hospital from January 2012 to December 2014 were selected as the research object and divided into research group(33 cases) and control group(31 cases)according to the different embryo culture and transplanting stages, patients with embryo culture and transplanting in blastocyst stage were regarded as the research group, patients with embryo culture and transplanting in cleavage stage were regarded as the control group, after the successful fertilization, they received early embryo culture with G1 and blastocyst culture with G2. Results There was 1 case cancelling the transplanting cycle in each group, and the actual numbers of transplanting cycle of the two groups were respectively 32 and 30, the differences in the embryo implantation rate and the pregnancy rate between the two groups were statistically significant(37.1% vs 19.4%)(53.1% vs 33.3%)(P〈0.05), the differences in the abortion rate and ectopic pregnancy rate by comparison were not statistically significant(P〉0.05), however, the multiple pregnancy rate in the control group was obviously higher than that in the research group(P〈0.05). Conclusion Embryo culture and transplanting in blastocyst stage can effectively improve the embryo implantation rate and the pregnancy rate, and can reduce the incidence rate of adverse pregnancy outcome and transfer cycle cancellation rate.
