摘要
目的通过对社区高血压患者契约式个性化管理,探讨契约式个性化管理模式的可行性。方法对辖区内350例患者根据血压和年龄分为对照组和实验组,根据不同的管理方法 (对照组采用高血压分级管理方法 ,实验组采用契约式个性化管理方法),分别进行为期一年的管理,对两组的体重、腰围及相关生化指标进行对比分析。结果经过干预,实验组的BMI、腰围、收缩压、舒张压均降低,差异有统计学意义;对照组这四组数据也有降低趋势但并无统计学意义。实验组中除了HDL升高外,其余三项指标均降低,差异有统计学意义;对照组四项指标虽都有所降低,但差异均无统计学意义。实验组限盐、控烟、坚持体育锻炼和控制饮食的人数都有显著性增加,依从性两组都无显著性差异。结论社区高血压患者的契约式个性化管理模式较我区现行的管理模式更有优势。
Objective To explore the feasibility of contract type personalized management mode, through the contract type personalized management of hypertension patients in community. Methods 350 patients were divided into the control group and the experimental group according to their blood pressure and age,and the management was conducted for one year according to the different management methods（The control group adopted the method of grading management of hypertension,the experimental group used the personalized management contract method）, then two groups of body weight, waist circumference, and the related biochemical indicators were compared and analyzed. Results After intervention, the BMI, waist circumference, systolic blood pressure and diastolic blood pressure in the experimental group were decreased, and there was statistical significance. The four groups of the control group also had a decreasing trend but no statistical significance. In the experimental group, in addition to HDL, the other three indicators were reduced,and all had statistical significance; while the control group of four indicators had been reduced, but there was no statistical significance. The number of limiting salt, tobacco control, adhere to physical exercise and diet in the experimental group significantly increased, compliance of the two groups had no significant difference. Conclusion The contract style personalized management mode of community hypertension patients is superior to the management mode of the first in our region.
