摘要
RCA^2是在RCA基础上引入行动(Actions),从而解决RCA疏于对解决方法的监督,最终无法有效避免不良事件或侥幸事件再次发生的方法。该方法的Actions是将通过RCA发现导致不良事件或侥幸事件的根本原因后,制定适宜的解决方法并监督落实,最终对效果进行评价和反馈,提高医院管理质量,更好地确保患者安全。
Root Cause Analyses and Actions （ RCA2 ） means add actions to the Root Cause Analyses （ RCA）, which is a method to solve the problem of the RCA method on supervision of countermeasures that cannot effectively prevent adverse events or these evens might happen again. After the discovery of the underlying causes for adverse events through RCA method, the actions of this method will develop appropriate solutions and oversee the implementation, finally evaluate and give feedback on the effec- tiveness, which improves the quality of hospital management, and better ensure patient safety.
