“五分法”2μm激光汽化切除术治疗BPH经验总结 被引量:6

“Five-part method” of two-micron continuous-wave laser vaporesection in treating benign prostatic hyperplasia

摘要 目的:总结"五分法"2μm激光汽化切除术治疗BPH的临床经验。方法:回顾性分析采用"五分法"2μm激光汽化切除术治疗BPH患者192例的临床资料。结果:所有患者手术均取得成功,手术时间为(88.43±24.51)min,术中出血量为(29.57±14.15)ml,术后冲洗时间为(4.15±2.44)h,拔除尿管时间为术后7天,术后出现继发性出血4例,出现暂时性尿失禁12例。术后随访3~12个月,无尿道狭窄等并发症发生。术后第3个月患者国际前列腺评分由术前(27.35±2.61)分降至术后(13.84±3.93)分(P〈0.05),生活质量评分由术前(4.93±0.96)分降至术后(1.86±1.11)分(P〈0.05),最大尿流率由术前(4.82±2.71)ml/s升至术后(16.84±3.37)ml/s(P〈0.05),膀胱残余尿量由术前(91.38±42.48)ml降至术后(24.12±16.69)ml(P〈0.05)。结论:"五分法"2μm激光汽化切除术是治疗BPH一种安全、有效的方法。 Objective:To summarize the clinical experience of "five-part method" of two-micron continuouswave laser vaporesection in treating benign prostatic hyperplasia(BPH).Method:A retrospective analysis was performed in 192 patients with BPH undergoing "five-part method" of two-micron continuous-wave laser vaporesection.Result:All operations were completed successfully.The average operation time was(88.43±24.51)min.The blood loss was(29.57±14.15)ml.The time of continuous bladder irrigation after operation was(4.15±2.44)h.Urinary catheters were indwelt for seven days.Four patients were found postoperative secondary hemorrhage,and 12 patients were found transient urinary incontinence.No urethral stricture or other complications occurred during the follow-up period of 3-12 months.The data of postoperative three months were as follows:IPSS decreased from(27.35±2.61)preoperatively to(13.84±3.93)postoperatively(P 0.05);quality of life(QOL)score decreased from(4.93±0.96)before operation to(1.86±1.11)(P 0.05);Qmaxincreased from(4.82±2.71)ml/s preoperatively to(16.84±3.37)ml/s postoperatively(P 0.05);bladder post void residual(PVR)decreased from(91.38±42.48)ml before operation to(24.12±16.69)ml after operation(P 0.05).Conclusion:Treatment of "five-part method" of two-micron continuous-wave laser vaporesection is safe and effective for BPH.
