摘要
目的探讨64排螺旋CT多平面重建和容积再现对肘关节恐怖三联征的诊断价值。方法搜集2013年1月~2015年6月在我院经手术证实的36例肘关节恐怖三联征患者,全部患者均行X线及螺旋CT检查,把MPR及VR图像与X线及横断面CT图像进行比较。结果 MPR联合VR图像对36例尺骨冠状突骨折、桡骨头骨折及肱尺关节脱位均能明确显示,诊断符合率为100%,高于X线及横断面CT图像;对发现尺骨冠状突及桡骨头骨折,三种方法比较,差异有统计学意义(P〈0.05);对发现肘关节脱位,三种方法比较,没有统计学意义(P〉0.05)。结论 64排螺旋CT的MPR、VR图像立体、直观,能全面显示肘关节恐怖三联征的影像学特征并进行准确分型。
Objective To assess the diagnostic value of multiplanar reconstruction(MPR)and volume rendering(VR)on64-row helical CT in terrible triad of the elbow.Methods The radiographs and CT of 36 patients with surgically proven terrible triads of the elbows from January 2013 to June 2015 were reviewed.The MPR and VR images were compared with radiographs and axial CT.Results MPR combined with VR imaging accurately demonstrated all 36 coronoid process and radial head fractures with elbow dislocation.The coincidence rate was 100%and was higher than that of radiography and axial CT.The differences between the three modalities in displaying the coronoid process and radial head fractures were significant(P〈0.05)whereas there was no significant difference in displaying the elbow dislocation(P〈0.05).Conclusion MPR and VR imaging on 64-row helical CT can accurately display the features of terrible triad of the elbow.
