摘要
目的探讨立体定向活检术在颅内诊断不明病变中的临床应用价值。方法回顾我科2009年1月-2015年6月收治的52例因颅内病变经影像学诊断不明的患者资料,均行立体定向活检术进行病理诊断。结果实性病变34例,囊性病变18例。实性病变中33例获得明确病理诊断,胶质母细胞瘤2例,间变性星形细胞瘤7例,少突胶质细胞瘤4例,转移瘤12例,淋巴瘤4例,恶性脑膜瘤1例,脱髓鞘病变2例,炎性病变1例,1例仍未获得明确病理诊断,活检阳性率97.1%;囊性病变中14例获得病理诊断,其中转移瘤7例,结核1例,黑色素瘤1例,脑脓肿5例,4例仍未获得明确明确病理诊断,活检阳性率77.8%。总活检阳性率90.4%。术中出现癫痫发作1例,术后癫痫发作1例,术后术区出血2例,颅内积气3例,无死亡病例。结论立体定向活检术是一种安全性高、病理诊断阳性率高的诊疗手段,尤其对实性病变病理阳性诊断率更高。对颅内不明原因病变进行立体定向活检定位精确、创伤小,在定性诊断中有重要意义,为下一步治疗提供循证学依据。
Objective To investigate the clinical application value of stereotactic biopsy in the diagnosis of unknown intracranial lesions.Methods 52 cases of patients with intracranial lesions were treated in our department from January 2009 to June 2015,The pathological diagnosis was performed by stereotactic biopsy.Results 34 cases of solid lesions,18 cases of cystic lesions.solid lesions in 33 cases obtained definite pathological diagnosis,2cases of glioblastoma,7cases of anaplastic astrocytoma,4cases of oligodendroglioma,12 cases of metastases,lymphoma in 4cases,1case of malignant meningioma and demyelinating lesions in 2cases,1cases of inflammatory lesions and 1case still did not get clear definite pathological diagnosis,The positive rate of biopsy was 97.1%;cystic lesions in 14 cases of pathological diagnosis was obtained,which metastatic tumor in 7cases,tuberculosis in 1case,1cases of melanoma,brain abscess in 5cases,4cases have not yet obtained clear definite pathological diagnosis,The positive rate of biopsy was 77.8%.The total positive rate of biopsy was 90.4%.1cases occurred epilepsy during operation.,1cases occurred epilepsy after operation,2cases of postoperative hemorrhage,3cases of intracranial gas,and no deaths.Conclusion Stereotactic biopsy is a safe and high diagnostic method,especially for the diagnosis of solid lesions with higher accuracy.It is important significance for the diagnosis of intracranial lesions,and is accurate and less trauma,To provide evidence for the next treatment.
