摘要
目的观察不同支架冠状动脉置入联合氯吡格雷治疗冠心病的效果及对血液流变学的影响。方法选择2014年4月—2015年6月陕西省咸阳市中心医院心内科住院接受经皮冠状动脉介入术(PCI)治疗的冠心病患者122例,按随机数字表法分为观察组61例和对照组61例,对照组采用PARTNER药物(雷帕霉素)洗脱支架联合氯吡格雷治疗,观察组采用Nano无载体含药(雷帕霉素)洗脱支架联合氯吡格雷治疗;比较2组患者心功能变化水平、血脂及CRP、血液流变学指标变化情况;术后随访不良反应发生情况。结果治疗后2组患者心功能、血脂及CRP、血液流变学各项指标均改善,且观察组改善程度优于对照组(P<0.05),术后心血管事件及出血等不良反应发生情况低于对照组,但差异无统计学意义(8.2%vs.4.9%,P>0.05)。结论 Nano无载体含药(雷帕霉素)洗脱支架联合氯吡格雷治疗冠心病,能有效改善患者心功能水平,提高血液流变学指标,减轻炎性反应,安全性好,值得临床推广应用。
Objective To explore the clinical efficacy of different coronary artery stent placement combined with clopidogrel and it＇s effect on blood theology in the treatment of coronary heart disease.Methods A total of 122 cases of patients with coronary heart disease who treated by percutaneous coronary intervention（PCI） in Department of Cardiology of Central Hospital of Xianyang City,Shanxi Province from April 2014 to June 2015 were randomly divided into observation group（61cases） and control group（61 cases）,the conlrol group was treated with PARTNER drugs（rapaniycin） eluting stent combined with clopidogrel,the observation group was treated with Nano unsupported containing drugs（rapaniycin） eluting stent combined with clopidogrel,the changes of heart function,the changes of blood lipid index,CRP and blood rheology,and the incidence of adverse reactions after operation was compared.Results After treatment,the indexes of heart function,blood lipid,blood rheology and CRP were improved,and the patients in the observation group were better than the control group（P〈0.05）,and the incidence of postoperative cardiovascular events and bleeding were lower than those in the control group（8.2%vs.4.9%,P〈0.05）.Conclusion Nano unsupported containing drugs（rapaniycin） eluting stent combined with clopidogrel in the treatment of coronary heart disease,can effectively improve the patient＇s heart function,improve blood rheology index,reduce inflammatory reaction,and it is worthy of clinical application with good safety.
