摘要
目的探讨腹膜透析授权简化量表（PDES-SF）在腹膜透析患者中应用的信效度。方法选取2015年3—5月天津市第一中心医院肾内科腹膜透析中心手工操作腹膜透析患者136例,采用专家函询法将糖尿病授权中文简化量表（C-DES-SF）改编成PDES-SF,对腹膜透析患者发放PDES-SF、慢性病管理自我效能量表（SES6）、腹膜透析患者自我管理量表（SMSPD）、自尊量表（SES）及心理一致感量表（SOC-13）进行问卷调查,同时收集同期腹膜透析患者的生化指标。结果腹膜透析患者PDES-SF得分为（4.1±0.7）分,胡贝贝等的糖尿病患者C-DES-SF得分为（3.9±0.7）分,差异有统计学意义（t=3.48,P=0.001）。效度分析得出PDES-SF最终的评价者间一致性水平（IR）为0.875,条目水平的内容效度指数（I-CVI）范围为0.80~1.00,平均量表水平的内容效度指数（S-CVI/Ave）为0.95,全体一致量表水平的内容效度指数（S-CVI/UA）为0.88。PDES-SF得分与SES6得分（r=0.549,P〈0.01）、SMSPD得分（r=0.414,P〈0.01）、SES得分（r=0.404,P〈0.01）、SOC-13得分（r=0.522,P〈0.01）均呈正相关;PDES-SF得分与清蛋白（r=0.216,P〈0.05）、全段甲状旁腺激素（i PTH）（r=0.224,P〈0.05）均呈正相关。信度分析得出PDES-SF Cronbach＇sα系数为0.851,重测信度r=0.963（P〈0.01）。结论 PDES-SF在腹膜透析患者中具有较好的信效度,适合测量腹膜透析患者的授权能力。
Objective To explore the reliability and validity of the peritoneal dialysis empowerment scale- short form（ PDES-SF） among peritoneal dialysis patients. Methods A total of 136 patients who conducted peritoneal dialysis by manual manipulation in peritoneal dialysis center of Nephrology Department of Tianjin First Central Hospital were enrolled from March to May in 2015. Chinese diabetes empowerment scale- short form（ C-DES-SF） was adapted into PDES-SF by expert enquiry; the patients with peritoneal dialysis were surveyed by PDES-SF,self- efficacy for managing chronic disease 6 item scale（ SES6）,self- management scale for peritoneal dialysis patients（ SMSPD）, self- esteem scale（ SES） and sense of coherence- 13（ SOC-13）. And the biochemistry indicators of patients with peritoneal dialysis were collected in the corresponding period. Results The PDES-SF score of the patients with peritoneal dialysis was（ 4. 1 ± 0. 7） and the C-DES-SF score of diabetes patients including Hu Beibei was（ 3. 9 ± 0. 7）; the difference was significant（ t = 3. 48,P = 0. 001）. The consistency level（ IR） between ultimate valuators about PDES-SF was 0. 875 by validity test; the range of content validity index（ I-CVI）about level of entries was 0. 80- 1. 00; the scale- level content validity index / average（ S-CVI / Ave） was 0. 95; scale- level content validity index / universal agreement（ S-CVI / UA） was 0. 88. The PDES-SF score was positively correlated with SES6（ r= 0. 549,P〈0. 01）,SMSPD（ r = 0. 414,P〈0. 01）,SES（ r = 0. 404,P〈0. 01） and SOC-13（ r = 0. 522,P〈0. 01）score,albumin level（ r = 0. 216,P〈0. 05） and intact parathyroid hormone（ i PTH）（ r = 0. 224,P〈0. 05）. PDES-SF Cronbach＇s α coefficient was 0. 851 and test- retest reliability was 0. 963 by reliability test. Conclusion PDES-SF has good reliability and validity and is suitable for measuring the empowerment ability of peritoneal dialysis patients.
