氧气流量和湿化方式对重型颅脑损伤气管切开患者痰液流变学的影响研究 被引量:2

摘要 目的:探讨氧气流量和湿化方式等因素对重型颅脑损伤气管切开患者痰液流变学的影响。方法:回顾性分析76例重型颅脑损伤气管切开患者,按随机抽样法分为A组和B组,每组各38例。A组患者给予沐舒坦雾化吸入治疗;B组患者在A组基础上用氯化钠注射液持续气道内滴入。用Low-Shear 40流变仪对两组患者的痰液进行黏弹性变化的比较分析。结果:对照研究发现,两组患者的吸氧流量、吸氧时间长短、气道湿化方式都对痰液的黏性、弹性差异有统计学意义(P<0.05)。调整给氧流量和人为气道管理,确实可以降低两组患者的痰液流变学特性,有效减少患者肺部并发症的发生。结论:吸入氧气流量越大、持续时间越长,痰液黏弹性越大。经过人为干预,湿化治疗能够降低气管切开患者的痰液黏弹性。 Objective: To explore effects of oxygen flow,humidification way and other factors on sputum rheology for sever craniocerebral injury patient with incision of trachea. Methods: Seventy- six severe craniocerebral injury patients with incision of trachea were chosen and divided into A group( n = 38) and B group( n = 38) according to the random sampling method. A group was given Mucosolvan aerosol inhalation treatment,while B group was dripped with Na Cl injection continuously in the air way based on that of A group. The changes of sputum viscoelasticity of the two groups were compared and analyzed with the low- shear 40 rheometer.Results: The comparison showed that there were statistical differences in the effects of the oxygen flow,the length of time for oxygen inhalation,and humidification way on the sputum elasticity and viscoelasticity between the two groups( P〈0. 05). Adjustment to the oxygen flow and artificial airway management could indeed reduce the sputum rheology and the incidence of pulmonary complications.Conclusions: The longer the flow- rate of oxygen is and the duration lasts,the greater the elasticity of sputum is. Through human intervention,the clinical treatment of humidification can reduce the viscoelastic sputum of the patients with tracheotomy.
