摘要
目的比较舒芬太尼与芬太尼复合小剂量布比卡因在泌尿外科腹腔手术麻醉中的应用效果。方法选取2013年3月~2015年3月在我院择期进行腰麻下泌尿外科手术治疗的80例患者作为研究对象,按入院顺序编号,随机分为A、B两组,各40例。A组采用舒芬太尼复合小剂量布比卡因腰麻方案,B组则改用芬太尼复合小剂量布比卡因,比较两组阻滞效果,统计两组麻醉不良反应。结果麻醉前,2组血流动力学对比差异无统计学意义(P〉0.05),T1、T2时间点,A组SBP、DBP均高于B组(P〈0.05);A组感觉阻滞起效时间(2.6±0.5)min,短于B组,感觉阻滞持续时间(112.6±19.8)min,长于B组,两组比较差异有统计学意义(P〈0.05),且其感觉阻滞平面高于B组;A组麻醉不良反应发生率为17.5%,B组为27.5%,A组稍低于B组,但对比差异无统计学意义(P〈0.05)。结论在泌尿外科手术麻醉中,采用舒芬太尼复合小剂量布比卡因方案,对人体呼吸系统影D响小,阻滞起效速度快,阻滞平面完善,值得应用。
Objective To compare the effects of sufentanil and fentanyl combined with low - dose bupivacaine in anesthesia of abdominal surgery in the department of urology. Methods 80 patients underwent surgical treatment un- der lumbar anesthesia in the department of urology in our hospital from Mar. 2013 to Mar. 2015 were selected. Patients were randomly divided into group A and group B according to the order of admission with 40 patients in each group. Patients in group A were given sufentanil combined with low - dose bupivacaine for lumbar anesthesia while patients in group B were given fentanyl. The block effects in the two groups were compared. The adverse reactions of anesthesia in the two groups were statistically analyzed. Results There were no significant differences in hemodynamics between the two groups before anesthesia(P 〉 0.05). SBP and DBP were higher in group A than those in group B at T1 and T2 ( P 〈 0.05 ). The onset time of sensory block in group A (2.6 ±0.5 ) min was shorter while duration of sensory block ( 112.6± 19.8) min was longer and its sensory block plane was higher than that in group B ( P 〈 0.05 ). The incidence of adverse reactions during anesthesia in group A ( 17.5 % ) was slightly lower than those in group B (27.5 % ) (P 〈 0. 05). Conclusion In urologic surgical anesthesia, sufentanil combined with low - dose bupivacaine solution has little influence on the respiratory system of human body. The onset time of block is short and the the block plane is complete.
