摘要
目的探讨24小时动态脑电图(AEEG)监测对癫痫患者的临床意义。方法选取我院神经内科2012年4月-2014年10月进行治疗的240例癫痫患者,均在我院进行了AEEG与常规脑电图(REEG)检查。明确诊断120例癫痫的患者为A组,经过反复检测并结合临床表现最终确认的癫痫120例患者为B组(夜间发作次数多于白天),分别对两组患者AEEG与REEG检查的异常率及痫样放电检出率进行对比分析,同时比较AEEG与REEG检查对240例患者异常率及痫样放电检出率。结果 A组患者REEG痫样放电检出率为19.2%,异常率为38.3%,AEEG痫样放电检出率为61.7%,异常率为70.8%,两种方法检测结果间差异均有统计学意义(均P〈0.05)。B组患者REEG痫样放电检出率为17.5%,异常率为36.7%,AEEG痫样放电检出率为52.5%,异常率为67.5%,两种方法检测结果间差异均有统计学意义(均P〈0.05)。240例癫痫患者中REEG痫样放电异常的总检出率为18.3%,总体异常检出率为37.5%,AEEG痫样放电总检出率为57.1%,总体异常率为69.1%,差异均有统计学意义(均P〈0.05)。结论 AEEG监测对癫痫患者的疾病诊断、病情监测、预后判断及临床用药治疗都有重要的临床应用价值。
Objective To investigate the clinical significance of 24 hours ambulatory electroencephalogram(AEEG)monitoring in patients with epilepsy.Methods 240 patients with epilepsy were treated with AEEG and conventional electroencephalogram(REEG)including group A(120patients with diagnosis for epilepsy)and group B(120patients with diagnosis after repeated testing combined with clinical manifestations for epilepsy).The AEEG with REEG check abnormal rate and epileptiform discharge detection rate were compared and analyzed.Results The epileptiform discharges REEG detection rate and abnormal rate of group A were 19.2% and 38.3%.The AEEG epileptic discharge detection rate and abnormal rate of group A were 61.7% and 70.8%.The difference had statistical significance(P〈0.05).The REEG epileptiform discharge detection rate and abnormal rate of group B were 17.5% and 36.7%.The AEEG epileptic epileptiform discharge rate and abnormal rate of group B were 52.5% and 67.5%.The difference had statistical significance(P〈0.05).The total detection rate and overall abnormal rate of abnormal REEG epileptiform discharges were 18.3% and 37.5%.The total detection rate and overall abnormal rate of abnormal AEEG epileptiform discharges were 57.1%and 69.1%.The difference of total abnormal rate had statistical significance(P〈0.05).Conclusion AEEG monitoring has important clinical value in diagnosis,disease monitoring and prognosis of epilepsy.
