摘要
目的探讨糖尿病肾病尿毒症患者尿路感染病原菌特点、感染的危险因素,为临床降低糖尿病肾病尿毒症患者尿路感染率提供依据。方法回顾性分析2012年8月-2015年6月医院收治的105例糖尿病肾病尿毒症患者临床资料,分析其病原菌分布特点,采用多因素logistic回归分析诱发尿路感染的相关危险因素,采用SPSS19.0软件进行统计分析。结果 105例糖尿病肾病尿毒症患者发生尿路感染31例,感染率为29.52%;共检出病原菌39株,其中革兰阴性菌、革兰阳性菌、真菌分别占58.97%、33.34%、7.69%;单因素分析发现,年龄>60岁、女性、糖尿病病程>10年、住院时间>20d、空腹血糖>8mmol/L及应用抗菌药物是患者发生尿路感染的危险因素(P<0.05);多因素logistic回归分析显示,年龄>60岁、女性、血清白蛋白≤3g/L、留置尿管是患者发生尿路感染的独立危险因素(P<0.05)。结论糖尿病肾病尿毒症患者尿路感染的危险因素较多,高龄、女性是发生尿路感染的高危人群,应采取相应措施加强营养和留置导管护理,以降低糖尿病肾病尿毒症患者尿路感染率。
OBJECTIVE To explore pathogen characteristics and risk factors of urinary tract infections in uremia patients with diabetic nephropathy,so as to provide evidence for reducing urinary tract infections.METHODS A total of 105 uremia patients with diabetic nephropathy from Aug.2012 to Jun.2015 in our hospital were collected,and their clinical data were retrospective analyzed.The distribution characteristics of pathogens were analyzed.Risk factors associated with urinary tract infections were analyzed by multivariate logistic regression.SPSS 19.0was explored to make statistical analysis.RESULTS Totally 31 patients got urinary tract infections among 105 uremia patients with diabetic nephropathy;the infection rate was 29.52%.And 39 strains of pathogens were detected,including gram-negative bacteria,gram-positive bacteria,and fungi,accounting for 58.97%,33.34%,and7.69% respectively.Univariate analysis showed that age60,females,duration of diabetes〉10years,hospitalization time〉20d,fasting plasma glucose 〉8 mmol/L,and use of antibiotics were risk factors for patients with urinary tract infections（P〈0.05）.Logistic regression analysis showed that age〉60,females,albumin ≤3g/L,indwelling catheter were independent risk factors in patients with urinary tract infection（P〈0.05）.CONCLUSIONThe results show that risk factors for urinary tract infections in uremia patient are various.Senior citizens,women are at high risk of urinary tract infections,and appropriate measures should be taken to strengthen nutrition and catheter care for reducing urinary tract infections rate in uremia patients with diabetic nephropathy.
