组织因子及其剪切体在胃癌组织中的表达及其预后的研究

Expression of tissue factor and its alternatively splice in the gastric carcinoma tissue and their correlation with prognosis

摘要 目的探讨组织因子(TF)及其剪切体在胃癌组织中的表达,并分析全长组织因子(flTF)及其剪切体(asTF)与胃癌患者预后的关系。方法采用实时荧光定量聚合酶链反应(FQ-PCR)方法检测52例胃癌组织及10例正常胃组织的flTF及asTF的基因表达水平并分析临床意义,应用Cox模型分析其与胃癌患者死亡的关联程度。结果胃癌组织flTFmRNA相对定量[7.45(0.34~33.68)]高于正常胃组织[3.00(1.36~5.02)],差异有统计学意义(P〈0.05)。胃癌组织asTFmRNA相对定量[0.88(0.07—26.00)]高于正常胃组织[0.33(0.03~0.97)],差异有统计学意义(P〈0.01)。flTF与asTF基因表达水平与性别、年龄、TNM分期、肿瘤大小、组织学分型和化疗敏感性等预后相关因素的关系差异均无统计学意义(P〉0.05)。Log—rank生存分析显示f1TF低表达组[33.83(25.24~42.43)个月]比高表达组[18.18(13.32~23.04)个月]平均术后生存时间显著延长15.66个月,差异有统计学意义(x^2=6.185,P〈0.05)。asTF低表达组[25.32(21.19~29.45)个月]比高表达组[20,50(12.51~28.48)个月]平均术后生存时间显著延长4.82个月,差异有统计学意义(x^2=4.604,P〈0.05)。多因素Cox模型分析显示,flTF及asTF均是胃癌独立的预后风险因素[风险比(HR)=6.03,95%可信区间(CU):1.02—35.71,P〈0.05]及(HR=10.74,95%CI:2.32~49.59,P〈0.01)。结论flTF及asTFmRNA在胃癌组织中的表达水平较正常组织显著上调,flTF及asTF的基因表达水平是胃癌患者死亡的高危因素。 Objective To investigate the expression of tissue factor (TF) and alternatively spliced tissue factor (asTF) in human gastric carcinoma tissue and their correlation with prognosis. Methods The expression of full length tissue factor (flTF) and asTF mRNA were detected by real - time fluorescence quantitative polymerase chain reaction (FQ -PCR) in 52 human gastric carcinoma cases and 10 healthy gastric tissue cases. The correlations between clinical data of patients with gastric carcinoma, flTF and asTF were analyzed. Multi - factor Cox model analyzed the risk of the death of gastric cancer patients. Results The relative expression of flTF mRNA [ 7.45 (0. 34 - 33.68) ] in human gastric carcinoma tissues was significantly higher than those in healthy gastric tissues [ 3.00 ( 1.56 - 5.02) ], P 〈 0. 05, as well as asTF mRNA [ 0. 88 (0. 07 - 26.00 ) ] in gastric carcinoma tissues than those in healthy gastric tissues [0. 33 (0. 03 -0. 97) ], P 〈0. 01. There was no correlation between the expression of flTF, asTF and relevant elements of prognosis such as gender, age, TNM stage, pathological grade, tumor size, histological type or chemotherapy sensitivity. Single factor analysis demonstrated that the average survival time of gastric carcinoma patients with flTF and asTF low expression was longer than those with high expression, which showedflTF {[33.83 (25.24-42.43)]vs. [18.18 (13.32-23.04)],x^2=6.185, P〈0.05) and asTF { [ 25.32 (21.19 - 29.45 ) ] vs. [ 20. 50 ( 12. 51 - 28.48 ) ], x^2 = 4. 604, P 〈 0. 05 }- Multi - factor Cox model analysis demonstrated that flTF [ hazard ratio (FIR) = 6.03, 95% confidence interval (CI) :1.02 - 35.71, P 〈 0.05 ] or asTF ( HR = 10.74, 95% CI: 2. 32 - 49. 59, P 〈 0. O1 ) expression may be considered to be the prediction of survival time in the gastric carcinoma patients, which showed flTF and asTF was an independent negative regulator for the prognosis of gastric cancer.
