肾素-血管紧张素系统关键基因交互作用对原发性高血压病中医证候的影响 被引量:9

Influence of interaction among key genes of RAS on TCM syndromes of essential hypertension

摘要 目的:探讨原发性高血压病(EH)患者不同中医证型与肾素-血管紧张素系统(RAS)关键基因血管紧张素转化酶(ACE)、血管紧张素原(AGT)、血管紧张素Ⅱ-1型受体(AT1R)基因交互作用的相关性。方法:选择EH患者400例,中医辨证分型为肝火亢盛型、阴虚阳亢型、阴阳两虚型、痰湿壅盛型,另选取对照组100例,检测其临床生化指标,采用多重高温连接酶技术(im LDR)检测ACE基因I/D、AGT基因G217A、AT1R基因A1166C基因多态性。结果:AGT基因G217A位点基因型在不同中医证型间分布差异具有统计学意义(P<0.05)。ACE基因I/D、AT1R基因A1166C位点基因型、等位基因在不同中医证型间频率分布差异均无统计学意义。ACE基因I/D与AGT基因G217A位点组成的交互模型是EH肝火亢盛组、痰湿壅盛组和阴阳两虚组的最佳模型(P<0.05)。结论:RAS关键基因之间的交互作用对EH中医证型的发病有影响;ACE、AGT基因之间存在协同作用,与EH肝火亢盛证、痰湿壅盛证和阴阳两虚证发病有关。 Objective: To discuss the correlation between TCM syndromes and key genes of renin-angiotensin system(RAS) which contains angiotensin converting enzyme(ACE) gene, angiotensinogen(AGT) gene and angiotensin Ⅱ type 1 receptor(AT1R) gene of patients with essential hypertension(EH). Methods: Four hundred EH patients were divided into syndrome of exuberance of liver fire group, syndrome of hyperactivity of yang due to yin deficiency group, syndrome of deficiency of both yin and yang group, and syndrome of excessive accumulation of dampness-phlegm group according to syndrome differentiation. Another 100 cases were included and served as the control group. The clinical biochemical indicators of patients in each group were detected. The polymorphisms genes as I/D of ACE gene, G217 A of AGT gene and A1166 C of AT1 R gene were detected by using improved multiple ligase detection reaction(im LDR). Results: The difference in distribution of G217 A genotype of AGT gene among different syndromes was statistically significant(P0.05). The difference in distribution of I/D genotype of ACE gene and A1166 C genotype of AT1 R gene among different syndromes was not statistically significant. The interactive model constitute by I/D genotype of ACE gene and G217 A genotype of AGT gene was the optimization model of EH patients in syndrome of exuberance of liver fire group, syndrome of deficiency of both yin and yang group, and syndrome of excessive accumulation of dampness-phlegm group(P0.05). Conclusion: The interaction among key genes of RAS could influence the TCM syndrome of EH. There is a synergistic effect between ACE and AGT genes which are related with the occurrence of EH with syndrome of exuberance of liver fire, syndrome of deficiency of both yin and yang, and syndrome of excessive accumulation of dampness-phlegm.
