摘要 目的:探讨胎盘MRI弥散加权成像(DWI)在单绒毛膜双胎妊娠选择性胎儿生长受限评估中的应用价值。方法:回顾性分析2013年12月至2015年6月期间于我院行产前MRI检查患者60例,其中选择性生长受限28例(S组),生长正常32例(N组),对影像图像进行分析,并对各组表观弥散系数(ADC)、r ADC采用配对t检验进行统计学分析。结果:S组与N组各个胎儿胎盘区ADC差异有统计学意义(P<0.05),S组小胎儿胎盘区ADC值显著小于其余各组(P<0.0001)。S组r ADC明显高于N组(P<0.05)。结论:DWI通过定量测量ADC值,能够发现选择性胎儿生长受限胎儿胎盘功能的障碍以及胎盘的异质性,不仅为s FGR的发病机制提供进一步的研究线索,而且能够为临床治疗提供可靠的依据。 Purpose:To discuss the value of diffusion weighted imaging(DWI) in the diagnosis of selective fetal growth restriction(s FGR)in monochorionic twins(MC twins). Methods: Sixty MC pregnant women who took the fetal MRI in our hospital between December 2013 and June 2015 were retrospectively analyzed. There were 28 cases of s FGR(S group), and 32 cases of normal growth(N group). The routine MRI results were reviewed and the ADC and r ADC between the two groups were compared by using student t test. Results: There was significant difference of ADC between the S group and the N group(P〈0.05), especially the ADC values of the smaller fetus in the S group were clearly smaller than that in any other groups(P〈0.0001). And the r ADC values of S group were higher than that of N group(P〈0.05). Conclusion: DWI could provide more valuable information about s FGR, and a decreased ADC may be used as an early marker of s FGR.
