慢性鼻窦炎患者FESS手术后引起嗓音改变机制的研究 被引量:2

Changes in Acoustic Parameters of Voice after Functional Endoscopic Sinus Surgery in Patients with Chronic Sinusitis

摘要 目的:研究慢性鼻窦炎患者FESS手术前后嗓音声学参数的改变。方法:回顾性分析90例接受FESS术的患者和30例正常人术前及术后6周的嗓音声学参数资料,根据诊断将病人分为A、B、C3组,同时将30名正常的人设为D组。运用嗓音分析软件进行嗓音分析,比较患者手术前后频率微扰、振幅微扰、谐噪比、基频的变化。结果:1FESS手术后,各组的基频同术前相比无明显区别。2双侧鼻息肉完全或接近完全阻塞的患者术后的Jitter、Shimmer、HNR变化有显著的差异,有统计学差别(P<0.05),而手术前后F0相比,差距无统计学差异(P>0.05)。结论:FESS手术前后共鸣腔的变化会引起患者嗓音的变化,且术前鼻腔阻塞越重,术后嗓音参数改变就越明显。 Objective: To evaluate acoustic voice parameters changes of patients diagnosed with chronic sinusitis before and after functional endoscopic sinus surgery. Methods: Ninety patients diagnosed with chronic sinusitis who had undergone functional endoscopic sinus surgery and thirty normal peoples were included in this retrospective study. Patients were divided into group A, B and C according to the diagnosis before the surgery, and thirty normal peoples were set to group D. Acoustic voice analyses were performed on each patient before the endoscopic sinus surgery and six weeks following the surgery with the voice program. Parameters like Jitter, Shimmer,F0 frequency, and harmonic to noise ratio(HNR) were obtained for acoustic analysis before and after operation. Results: 1 No statistically significant change in F0 was measured by acoustic analysis of voice after endoscopic sinus surgery. 2 The postoperative change in Jitter, Shimmer, HNR values was statistically significant in the patients with total nasal obstruction(P〈0.05), but there was no statistical difference in F0 between before and after operation(P〈0.05). Conclusion: The modifications in the anatomical structure of the resonance pre-and post-FESS would cause the voice changes of patients. And the more severe the preoperative nasal obstruction was, the more obviously the postoperative voice parameters changed.
