摘要
目的观察射频热凝术对针刺诱导的兔椎间盘退变髓核组织中一氧化氮(NO)、肿瘤坏死因子-α(TNF-α)及白细胞介素-1(IL-1)水平的影响。方法将30只兔随机分为3组:空白对照组(6只)、假手术组(6只)和造模组(18只)。空白对照组不行开放性手术;假手术组行开放性手术显露椎间盘,但不行造模处理;空白对照组、假手术组于当天以空气栓塞法处死兔后,切取L5/6椎间盘标本。采用纤维环穿刺法建立兔椎间盘退变模型。造模成功后,将造模组18只兔再随机分为3组:模型组、射频处理1组和射频处理2组,每组6只。模型组不行椎间盘射频热凝术,于当天以空气栓塞法处死兔后,切取L5/6椎间盘标本;射频处理1组行椎间盘射频热凝术,并于当天以空气栓塞法处死兔后,切取L5/6椎间盘标本;射频处理2组行椎间盘射频热凝术,射频后7d以空气栓塞法处死兔后,切取L5/6椎间盘标本。采用ELISA法检测各组上清液中NO、TNF-α和IL-1水平。结果与空白对照组、假手术组比较,模型组上清液中NO、TNF-α及IL-1水平均明显升高,射频处理1组、射频处理2组上清液中NO、TNF-α水平则明显升高(均P<0.05);与模型组比较,射频处理1组、射频处理2组上清液中NO、TNF-α及IL-1水平均明显降低(均P<0.05)。结论射频热凝能够减少兔退变椎间盘髓核组织NO、TNF-α和IL-1含量。射频热凝术后,椎间盘髓核组织中NO、TNF-α和IL-1含量均较稳定。
Objective To observe the effects of radiofrequency thermocoagulation on the levels of nitric oxide(NO),tumor necrosis factor-α(TNF-α)and interleukin-1(IL-1)in nucleus pulposus in rabbits with intervertebral disc degeneration induced by needle puncture.Methods Thirty rabbits were randomly divided into three groups.The blank control group(6rabbits)did not undergo open surgery.The sham-operation group(6rabbits)only received open surgery to expose the intervertebral disc.L5/6intervertebral disc samples were obtained in rabbits in blank control group and sham-operation group after sacrifice by air embolism.The intervertebral disc degeneration was induced by annulus puncture in other 18 rabbits.These rabbits with intervertebral disc degeneration were randomly divided into three groups:model group,radiofrequency treatment group1 and radiofrequency treatment group 2,with 6rabbits in each group.In model group,rabbits did not undergo radiofrequency thermocoagulation and L5/6intervertebral disc samples were obtained after sacrifice by air embolism.In radiofrequency treatment group 1,rabbits received radiofrequency thermocoagulation and L5/6intervertebral disc samples were obtained on the day of treatment.In radiofrequency treatment group 2,rabbits received radiofrequency thermocoagulation and L5/6intervertebral disc samples were obtained 7days after treatment.The levels of NO,TNF-αand IL-1in nucleus pulposus tissues were detected by ELISA.Results Compared with blank control group and sham-operation group,a significant increase was found in the levels of NO,TNF-αand IL-1in model group,as well as in the levels of NO and TNF-αin radiofrequency treatment group 1and radiofrequency treatment group 2(P〈0.05).Compared with model group,the levels of NO,TNF-αand IL-1decreased in radiofrequency treatment group 1and radiofrequency treatment group 2(P〈0.05).Conclusion Radiofrequency thermocoagulation can reduce and stabilize the contents of NO,TNF-αand IL-1in disc nucleus pulposus in rabbits with
