摘要
目的观察合并阻塞性睡眠呼吸暂停综合征(OSAHS)老年冠心病患者冠状动脉病变特点及影响因素。方法冠心病合并OSAHS患者64例,根据睡眠呼吸暂停低通气指数分为轻度OSAHS组(22例),中度OSAHS组(24例),重度OSAHS组(侣例)。单纯冠心病组32例。应用Syntax评分确定冠状动脉病变程度,比较4组间血压、体重指数(BMl)、吸烟指数、低密度脂蛋白胆固醇(LDL-C),血葡萄糖(GLU)、血hs-CRP、IL一6、TNF-α。结果冠心病合并OSAHS组BMI、SBP、Syntax分值、hs-CRP、IL-6、TNF-α、吸烟指数、LDL-C均高于单纯冠心病组(P<0.01或0 05),且随着OSAHS程度加重而增大。多元线性逐步回归分析显示上述参数均与Syntax分值存在相关性。结论老年冠心病患合并OSAHS患者冠状动脉病变较重,并与低通气、吸烟及炎症密切相关。
Objective To analyze the characteristics of coronary lesions and influence factors in elderly patients with coronary heart disease （CAD） and obstructive sleep apnea syndrome （OSAHS）. Methods A total of 64 patients with both CAD and OSAHS were divided into mild OSAHS group （n=22）, moderate OSAHS group （n=24） and severe OSAHS group （n=18）. 32 patients with only CAD served as control group. Syntax score was used to evaluate the severity of coronary lesions. Blood pressure, body mass index （BMI）, smoking index, low density lipoprotein cholesterin （LDL- C）, glucose （GLU）, hsCRP, IL- 6 and TNF- α were compared between groups. Results BMI, SBP, Syntax score, hs- CRP, IL- 6, TNF- α, smoking index and LDL- C were significantly higher in patients with both CAD and OSAHS than in control group, and increased as the exacerbation of OSAHS. Multivariable regression analysis revealed that BMI, SBP, hs- CRP, IL- 6, TNF- α, smoking index and LDL- C were correlated with Syntax score. Conclusion Coronary artery lesions are severe in elderly patients with both CAD and OSAHS, and closely related to low ventilation, smoking and inflammation.
