摘要
目的：分析探讨腹腔镜胆囊切除胆管损伤的效果并进行分析。方法选择2010年1月至2014年12月在大同市三医院接受腹腔镜胆囊切除、发生胆管损伤治疗的患者23例，对其临床资料进行回顾性分析。结果23例患者中20例患者修复损伤成功，3例患者失败。1例患者因胆漏手术失败，1例患者因T管支撑移位失败，1例患者因吻合口早期狭窄失败，3例手术失败患者经2～3次手术后，采用胆肠内引流手术修复成功。23例患者无死亡病例。结论腹腔镜胆囊切除胆管损伤时，应充分了解患者的病变部位解剖结构，掌握合理的手术时机，手术操作时应认真细致，要正确处理手术中的危急情况，能有效减少腹腔镜胆囊切除胆管的损伤，降低患者痛苦。
Objective To analyze the effects of laparoscopic cholecystectomy on bile duct inju-ry.Methods Twenty-three laparoscopic cholecystectomy bile duct injury patients from January 2013 to December 2014 in the third hospital of Datong were selected , the data of the patients were retrospectively analyzed.Results Among the 23 patients, 20 patients successfully repaired the damage , 3 patients failed.One patient failed due to bile leakage surgery , 1 patient failed because of T tube support displace-ment , 1 patient failed because of early anastomotic stricture , 3 failure patients after 2 to 3 operation , bili-ary drainage surgery successful repaired the bile duct injury .There was no deaths .Conclusions When bile duct injury occurs in laparoscopic cholecystectomy ,we should fully understand the anatomy of the pa-tient’ s lesion, grasp the timing of surgery , the operation should be carefully to properly handle the situa-tion threatening surgery , it can effectively reduce bile duct injury during laparoscopic cholecystectomy , reduce the suffering of patients .
