青少年髌骨骨软骨骨折16例的临床分析 被引量:3

摘要 目的 分析应用可吸收钉固定治疗青少年髌骨骨软骨骨折的临床效果。方法 回顾性分析201 1年7月-2013年7月在新乡医学院第一附属医院应用可吸收钉固定治疗的髌骨骨软骨骨折的16例青少年患者,所有患者均行切开复位可吸收钉内固定髌骨骨软骨骨折,术后铰链支具固定6周。随访骨软骨骨折愈合及膝关节功能、髌骨脱位复发情况。结果 其中15例患者获得随访(1例初次脱位者失访),随访时间20-47个月。15例患者髌骨骨软骨骨折均骨性愈合,并获得了无痛且活动度满意的膝关节功能,按Insall评分标准评价髌股关节功能,优9例,良6例,优良率为100.00%,15例均无髌骨再脱位出现,15例患者术前Lysholm评分平均为(39.73±4.84)分,术后Lysholm评分平均为(84.35±6.28)分,术后Lysholm评分较术前明显改善,差异有统计学意义(P〈0.05)。结论 青少年的髌骨骨软骨骨折多涉及软骨下骨,对内固定有较强的把持力,更适合内固定。应用可吸收钉固定,无需二次手术,疗效满意。 Objective To analyze the clinical treatment efficacy of patellar osteochondral fracture in adolescent by bioabsorbable pins. Methods From July 2011 to July 2013, in the First Affiliated Hospital of Xinxiang Medical University, 16 cases of adolescent patients suffered with patellar osteochondral fracture treated by application bioabsorbable pins fixing was retrospective analyzed, All patients underwent open reduction osteochondral fracture of the patella by bioabsorbable pins and immobilization hy tension, hinged brace for 6 weeks postoperation. Osteochondral fracture healing, knee function and patellar dislocation recurrence were observed on follow-up. Results 15 patients were followed-up (1 case lost) and the follow-up time 20-47. months. All patients were healed, and obtained the satisfactory knee function. According to Insall score standard, the excellent and good rate was 100.00% (9 cases were excellent, 6 cases were good). According to Lysholm score standard, the mean postoperative socre [(84.35±6.28) scores] was more significantly increased than the mean preoperative socre [(39.73±4.84) scores], the difference was statistically significant (P 〈 0.05). Conclusion Patella cartilage fractures involving subchondral bone after patella dislocation in adolescents is more suitable for internal fixation. Absorbable nails were satisfactory for patellar cartilage fracture.
