超声血流向量成像评价舒张期左心室心肌致密化不全流体能量损耗 被引量:3

Vector Flow Mapping in Quantitative Evaluation on Left Ventricular Energy Loss During Diastole in Patients with Left Ventricular Non-compaction

摘要 目的应用超声血流向量成像(vector flow mapping,VFM)技术可视化观察左心室心肌致密化不全(left ventricular non-compaction,LVNC)患者左心室心腔内血液流场状态,量化评价舒张期左心室心腔内能量损耗(energy loss,EL)。方法选取62例(P<0.05)LVNC患者为观察组(LVNC组),其中临床组(LVNC1组)37例,亚临床组(LVNC2组)25例,健康对照组61例。采集3个完整心动周期标准心尖三腔彩色多普勒实时动态图像,在机测量左心室舒张及收缩功能参数。将图像导入专用工作站,根据时间流量曲线(time-flow curve,T-F curve)确定全舒张期(P0)、等容舒张期(P1)、舒张早期(P2)、快速充盈期(P3)、减慢充盈期(P4)、心房收缩期(P5)以及舒张末期(P6)并测量对应时相左心室内EL、涡旋面积和涡旋循环强度。比较各组舒张功能参数及EL值,比较LVNC1组与LVNC2组非致密层(Noncompaction,NC)心肌以及致密层(compaction,C)心肌厚度。将EL与涡旋面积、涡旋循环强度、左心室舒张功能参数进行相关性分析。结果 1EL值:LVNC组与健康对照组比较,P2、P3、P4、P0期EL值增高,差异有统计学意义(P<0.05)。LVNC1组与健康对照组比较,P1、P2、P3、P4、P0期EL值增高,差异有统计学意义(P<0.05)。LVNC2组与健康对照组比较,P0-P6期EL值增高,差异无统计学意义(P>0.05)。LVNC1组与LVNC2组比较P3、P5、P0期EL值增高,差异有统计学意义(P<0.05)。2NC、C、NC/C值:LVNC1组与LVNC2组比较,NC、NC/C值增高,C值降低,差异有统计学意义(P<0.05)。3相关性分析:健康对照组P3-EL值与E、E/A、e、e/a值呈正相关(P<0.05),与A值呈负相关(P<0.05);P5-EL值与A、a、E/e值呈正相关(P<0.05),与E/A、e、e/a值呈负相关(P<0.05)。LVNC组P3-EL值与E、E/A、e、e/a值呈正相关(P<0.05);P5-EL值与A、a、E/e值呈正相关(P<0.05),与E/A、e、e/a值呈负相关(P<0.05)。健康对照组P0-EL值与涡旋循环量呈正相关(P<0.05),与涡旋面积呈负相关(P<0.05);LVNC组P0-EL值与涡� Objective To assess left intraventricular blood flow patterns in patients with left ventricular non-compaction (LVNC) and explore the value of left ventricle energy loss (EL) for diastolic dysfunction using vector flow mapping(VFM). Methods Totally 62 patients with LVNC as case group and clinical change group (LVNC1) in 37 cases, subclinical group (LVNC2) in 25 cases,61 cases of healthy control group. Acquired standard dynamic apical 3 chambers Doppler flow images for 3 completed cardiac cycles for the measurement of general parameters of left ventricular function. Using off-line workstation for ac- quiring the EL,vortex area and circulation. Divid the Diastole of left ventricle into the isovolumic relaxation phase (P1), early dias- tolic phase( P2), rapid filling phase( P3 ), slow filling phase ( P4), atria contract period ( P5 ), late diastolic phase ( P6 ) and total diastolic phase (P0) based on time - flow curve ( T - F curve ). The general parameters , EL value , noncompactiou ( NC ) or com- paction (C)myocardial thickness and the correlation between EL and diastolic function parameters ,vortex area and circulation a- bove the groups were compared. Results (~)EL value: Compared between LVNC group and normal group, the EL of all periods were higher with statistical significance in P2, P3, P4, PO phases ( P 〈 0.05 ). Compared between LVNC 1 group and normal group, the EL of all periods were higher with statistical significance in P1, P2, P3,P4,P0 phases ( P 〈 0.05). Compared between LVNC2 group and normal group, the EL of all periods were higher but the difference was not statistically significant ( P 〉 0.05 ). Compared between LVNC1 group and LVNC2 group, the EL of all periods were higher with statistical significance in P3, P5, P0 phases (P 〈 0.05). NC ,C ,NC/C value: Compared between LVNC1 group and LVNC2 group,the NC,NC/C value were higher and tile C value were lower with statistical significance (P 〈0.05 ). C
