摘要
目的多层螺旋CT(MSCT)静态图像联合超声动态影像观察健康成年人不同喉运动状态下的喉关节结构。方法选择3名健康志愿者行低剂量CT扫描参数预实验,以确定能清晰显示并测量喉部解剖结构标志的最低剂量设置。另选16名健康志愿者在平静呼吸、深吸气、Valsalva呼吸、发"yi"音4种不同喉运动状态下分别进行喉部低剂量MSCT扫描重建联合超声检查。采用MSCT静态图像观察并测量不同喉运动状态下喉部软骨结构,超声动态影像观察不同喉运动状态下喉部的运动情况。结果 70 m A低剂量MSCT扫描图像可清晰显示喉软骨解剖结构标志。超声动态扫描的清晰度尚不足以精确测量,但可观察到喉动态活动。MSCT测量结果显示,与平静呼吸相比,深吸气时两侧声带突间距、肌突间距轻微缩小,但差异无统计学意义(P>0.05);Valsalva呼吸、发"yi"音时两侧声带突间距、肌突间距均较平静呼吸时明显缩小(P<0.05);Valsalva呼吸与发"yi"音两组间的各参数差异均无统计学意义(P>0.05);4种运动状态的正中矢状面环甲间隙差异无统计学意义(P>0.05)。结论 MSCT联合超声扫描能动态观察喉关节结构变化,对传统解剖学的静态观察是良好补充,并有望成为诊断喉疾病的新指标。
Objective To observe the structure of laryngeal joints in the different laryngeal motion states by static image of multislice spiral computed tomography (MSCT) combined with dynamic image of ultrasound for healthy adults. Methods Three healthy volunteers were enrolled in the pre-experiment for low-dose CT settings to determine the minimum dose for clearly displaying and measuring the laryngeal anatomic landmarks. Another 16 healthy volunteers underwent low-dose laryngeal MSCT scans and reconstruction combined with uhrasonogTaphy in the four motion states of eupnea ,deep inspiration ,Valsalva breathing and pronouncing 'yi'. The anatomical measurements of laryngeal cartilage structure were conducted in the different laryngeal motion states using static image of MSCT,and the laryngeal motions in the different laryngeal motion states were observed using dynamic imaging of ultrasound. Results The images scanned by MSCT with a low-dose of 70 mA could clearly display the laryngeal cartilage anatomic landmarks. The dynamic imaging of ultrasound was not clear enough for precise measurement,but could be use for observing the laryngeal dynamic motion. The results of MSCT measurement showed that the distances of both vocal processes and muscular processes when deep inspiration were shorter compared to the distances when eupnea, but there was no significant difference ( P 〉 0.05 ). The distances of both vocal processes and muscular processes were significantly shorter in the state of Valsalva breathing or pronouncing 'yi' compared to the distances when eupnea( P 〈 0.05 ). No statistical difference in all parameters was observed between Valsalva breathing and pronouncing 'yi' (P 〉 0.05 ). There was no statistical difference in the ericothyroid space on mid sagittal plane among 4 motion states ( P 〉 0.05 ). Conclusion MSCT combined with ultrasonic imaging can be used to dynamically observe the changes in the structure of laryngeal joints. It is a good complement for static observation on
