时间-空间相关成像技术结合M型超声评价妊娠期高血压疾病胎儿心室收缩功能的临床应用研究 被引量:11

Study on the clinical application of spatio-temporal imaging technology combined with M-mode ultrasound in evaluation of ventricular contractile function of fetuses born by mothers with hypertensive disorder complicating pregnancy

摘要 目的 探讨时间-空间相关成像技术结合M型超声(Spatio-temporal image correlation with M-mode display,STIC-M)在妊娠期高血压疾病胎儿心室收缩功能评价中的应用价值。方法 选择孕20~40周妊娠期高血压疾病孕妇与血压正常孕妇为研究对象。妊娠期高血压疾病孕妇根据高血压严重程度分为妊娠期高血压组(A组)、轻度子痫前期组(B组)和重度子痫前期组(C组);血压正常孕妇为对照组(D组)。应用STIC-M测量胎儿左、右心室缩短分数(Fractional shortening,FS),比较各组胎儿左、右心室FS值。结果 A组21例、B组25例、C组22例、D组34例纳入比较分析,C组胎儿右室FS值较其余各组降低(P〈0.05),其余各组之间右室FS值两两比较,差异无显著性(P〉0.05),4组胎儿左室FS差异均无显著性(P〉0.05)。结论 STIC-M能够通过测量胎儿FS监测胎儿心室收缩功能,可作为评价胎儿心室收缩功能的新方法。妊娠期高血压疾病胎儿在重度子痫前期右心室收缩功能降低,在这一时期评估胎儿右心室功能、及早发现心功能异常具有十分重要的临床意义。 Objective To explore the application value of spatio-temporal imaging technology combined with M-mode ultrasound in e- valuation of ventricular contractile function of fetuses born by mothers with hypertensive disorder complicating pregnancy (HDCP) . Meth- ods HDCP pregnant women and normotensive pregnant women during 20-40 gestational weeks were selected. HDCP pregnant women were divided into gestational hypertension group (group A), mild preeclampsia group (group B), and severe preeclampsia group (group C) ac- cording to the severity of hypertension. Normotensive pregnant women were enrolled into control group (group D) . Fetal fractional shortening (FS) values of both left ventricule and right ventricule were measured by spatio-temporal imaging technology combined with M-mode ul- trasound, then the results were compared. Results Twenty-one cases in group A, 25 cases in group B, 22 cases in group C, and 34 cases in group D were enrolled in this study. FS value of right ventricule in group C was statistically significantly lower than those in the other groups (P〈0. 05), but there was no statistically significant difference in FS value of right ventricule among the other groups (P〉0. 05 ) . There was no statistically significant difference in FS value of left ventricule among the four groups (P〉0. 05) . Conclusion Spatio-tempo- ral imaging technology combined with M-mode ultrasound can monitor fetal ventricular contractile function through measuring fetal FS values, which can be used as a new method to evaluate fetal ventricular contractile function. Contractile function of right ventricular decreases in fetuses born by mothers with severe preeclampsia, evaluating fetal right ventricle function during this period and timely finding fetal heart dys-function have important clinical significance.
