摘要
目的研究分步延期有限内固定联合支架外固定治疗高能量Pilon骨折的疗效。方法选择2012年4月-2014年8月期间在我院骨科就诊的80例高能量Plion骨折患者进行研究,根据治疗方法不同分为两组,延期组接受延期有限内固定结合外固定支架治疗,常规组接受急诊切开复位内固定治疗,比较两组患者的术后恢复情况、关节功能、并发症情况。结果 （1）恢复情况：与常规组比较,延期组患者的术后软组织肿胀消退时间、影像学完全愈合时间、完全负重下地时间较短;（2）关节功能：术后6个月、9个月、12个月时,延期组患者的Baird踝关节评分高于常规组;（3）并发症：延期组患者发生创伤性关节炎、骨不连以及踝关节僵硬的例数少于常规组。结论分步延期有限内固定联合支架外固定治疗能够促进骨折愈合、减少软组织肿胀、预防并发症、改善踝关节功能,是治疗高能量Pilon骨折的理想方法。
Objective To study the efficacy of stepwise delayed limited internal fixation combined with stent external fixation in treating high energy Pilon fractures. Methods 80 cases of high-energy Plion fractures patients in our hospital from April 2012 to August 2014 in orthopedic department of our hospital were choosed for study and randomly divided into two groups according different treatment methods.Delayed group receivedstepwise delayed limited internal fixation combined with stent external fixation,conventional group received emergency open reduction and internal fixation.Then postoperative recovery,joint function, complications were compared. Results（1）recovery situation： Compared with conventional group,postoperative soft tissue swelling subsided time, radiologic complete healing time,full weight bearing time of delayed group were shorter;（2）joint function：3,6,9 months after surgery,Baird ankle score of delayed group were higher than conventional group;（3）Complications：cases of traumatic arthritis,bone nonunion,anklejoint stiffness of delayed group were less than conventional group. Conclusion Stepwise delayed limited internal fixation combined with stent external fixation is helpful to promote healing,reduce soft tissue swelling,prevent complications and improve ankle function;it＇s an ideal method of high energy Pilon fractures.
