摘要
目的检测酒糟鼻患者胃黏膜中幽门螺旋杆菌(Hp)基因,探讨Hp基因的作用机制。方法选取165例酒糟鼻患者按照症状以及病理发展阶段划分红斑毛细血管扩张组、丘疹脓疱组和增生肉芽肿组,每组各55例,并将55例健康体检者作为对照组。所有对象均采用胃镜取胃黏膜组织,分离培养Hp,提取Hp基因DNA,并进行多聚酶链-限制性片段长度多态性(PCR-RFLP)基因分析方法检测;ELISA检测IL-8和IL-1水平。结果实验组的幽门螺杆菌毒力基因CagA、VacA、glmM、cagE、babA2、babB、iceA、lec1、lec2、dupA(jhp917和jhp918)、jhp933、jhp947的阳性率明显高于对照组,差异均具有统计学意义(P<0.05),且3组酒糟鼻患者之间差异也具有统计学意义(P<0.05),随着酒糟鼻的不断严重Hp毒力基因的阳性率越高。3组酒糟鼻患者IL-8和IL-1的水平与对照组比较差异均有统计学意义(P<0.05),而且红斑毛细血管扩张组、丘疹脓疱组和增生肉芽肿组之间比较差异也有统计学意义(P<0.05),酒糟鼻的病情越严重,IL-8和IL-1的水平越高。结论 Hp基因在不同病理类型的酒糟鼻患者胃黏膜中表现不同。Hp毒力基因可能上调了炎症介质IL-8和IL-1的表达,进而参与诱导酒糟鼻的发生发展。
Objective To detect Helicobacter pylori gene in gastric mucosa of patients with rosacea, and to study the mechanism of helicobacter pylori. Methods A total of 165 patients with rosacea were divided into three groups according to the symptoms and pathological stage: erythema capillaries expansion group (n=55), pimples pustules group (n= 55), hyperpla- sia granuloma group (n= 55), and 55 cases as a control group. Gastric mucosal tissue was taken from all the subjects, helico- bacter pylori was separated and cultivated, and its DNA was analyzed with polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP). IL-1 and IL-8 were analyzed by Elisa. Results The helicobacter pylori CagA, VacA virulence genes, glmM, cagE, babA2, new, ieeA, lecl, lec2, dupA (jhp917 andjhp918)gene, jhp933, jhp947 ratio of experimental group were significantly higher than the control group, with statistical significance (P〈0.05). The difference between the three groups of rosacea patients was statistically significant (P〈0.05), and the positive rate of Hp virulence genes increased with continuous severe rosacea. The differences of IL-8 and IL-1 levels in the three groups of rosacea patients and control were statistically significant (P〈0.05). The differences between erythema telangiectasia group, pustules group and granuloma hy- perplasia group were also statistically significant (P〈0.05), and the positive rate of Hp virulence genes increased with continu- ous severe rosacea. Conclusion Hp gene in different pathological types of rosacea is different. Rosacea is probably one of the symptoms caused by inflammatory mediators of IL-8 and IL-1 that induced by Hp virulence gene.
