摘要
目的探讨多排螺旋CT各种图像后处理技术对肝动脉的解剖变异显示的优缺点及临床应用价值。方法收集我院2013-01-2015-07行腹部CT增强扫描或腹部动脉CTA检查的患者120例,观察腹腔动脉、肝固有动脉、肝右动脉、肝左动脉的起源及分支情况,分别进行容积再现(VR)、最大密度投影(MIP)、多平面重组(MPR)、曲面重组(CPR)后处理,并比较不同后处理方法。结果 120例中111例(92.5%)有典型肝动脉分布。肝动脉变异9例(7.5%),其中肝右动脉变异4例(3.3%),肝左动脉变异1例(0.9%),肝固有动脉变异4例(3.3%)。各种常用图像后处理技术中,MIP既能清晰显示动脉的解剖学形态,又能显示病灶情况。VR能完整显示动脉的解剖学形态,在迷走血管和重叠血管的显示方面优于MIP,但无法观察管腔内情况。MPR无法完整显示动脉的解剖学全貌,但可显示管壁的钙化、斑块、管腔狭窄等。CPR可将迂曲的动脉伸展,显示较直观,并可进行管腔的测量。结论多排螺旋CT能直观、立体显示肝动脉的解剖变异。各种后处理技术中,VR和MIP两者联合应用对肝动脉的显示最佳;MPR及CPR也有很大的帮助。
Objective To investigate the merit and demerit of various CT post-processing technologies in observing the anatomical variation of hepatic artery and explore their clinical application values. Methods A total of 120 patients examined by abdominal enhanced CT scanning or abdominal artery CTA in our hospital from January 2013 to July 2015 were collected. The origin of celiac artery,proper hepatic artery,right hepatic artery,left hepatic artery and its branches were observed. The post-processing technologies of volume reformation( VR),maximum intensity projection( MIP),multiplanar reformation( MPR),curved reformation( CPR) were performed respectively and the effectiveness of different post-processing technologies was compared. Results Typical distribution of the hepatic artery was found in111 cases out of 120 cases( 92. 5%). There were 9 cases( 7. 5%) of hepatic artery variation,including 4 cases( 3. 3%) of right hepatic artery variation,1 case( 0. 9%) of left hepatic artery variation and 4 cases( 3. 3%) of proper hepatic artery variation. MIP displayed clearly the anatomical morphology and the lesions of artery. VR displayed the arterial anatomical morphology,and had a advantage in showing the aberrant vessels and overlapping vessels over MIP,but it did not display the intraluminal states. MPR displayed uncompletely the anatomical morphology,but it clearly displayed calcifications and plaques of the wall and stenosis of the luminal. CPR displayed more intuitively the stretched circuitous artery,and the lumen were measured. Conclusion Multi-slice spiral CT can display anatomical variations of hepatic artery intuitively and stereoscopically. The combined application of both VR and MIP could best display the hepatic artery,and MPR and CPR are also of great help.
