摘要
针对"如何实现证候规范化与辨证灵活性的统一"关键科学问题,研究构建出血性中风急性期证候规范化与辨证灵活性相结合的中医辨证模式——先辨别并规范有限、清晰的病机证素,再组合为复杂多样的证名。根据大样本临床流行病学调查结果——各种病机证素兼夹组合的频次、概率;参考有关文献资料,尤其是各种已有的证候诊断标准;结合临床实际,确立出血性中风急性期证候分类及其命名原则。通过对1 028例出血性中风急性期患者进行病机证素的流行病学调查,多种病机证素的兼夹组合可形成48种证,其中二证组合15种,三证组合15种,四证组合13种,五证组合4种,六证组合1种。
The flexibility of symptom differentiation induce the complexity of the syndrome,and it is rather difficult to standardize and unify the syndrome.We try to establish a new TCM mode combining the standardized syndrome diagnostic criteria with the flexibility of symptom differentiation on acute hemorrhagic stroke.The key to solve scientific issue was based on the concept ’elements of syndrome and pathogenesis（ESP）’.Pathogenesis syndrome elements derived from pathological factors.Because of the limited number and relatively clear content,pathogenesis syndrome factors combined into a complex and diverse certificate was entirely feasible to establish standardized syndrome diagnostic criteria.We got the results of large sample of clinical epidemiology from studying combined frequency of various ESP,referring to relevant information,combining with clinical practice.According to these results,we established the syndrome classification and nomenclature on acute phase of hemorrhagic stroke.1028 patients were investigated and analyzed.There were 48 types of combinations of ESP.Among them,two different ESP can be combined into 15 types,three different ESP can be combined into 15 types,four ESP were 13 types,five ESP were 4types,six ESP was only 1.
