摘要
目的探讨儿童急性淋巴细胞白血病(ALL)与母亲怀孕期间溶剂、油漆、石油接触及吸烟的关系。方法检索Pubmed、Embase、the Cochrane library和Web of Science数据库中的相关文献,对母亲孕期接触苯和儿童急性淋巴细胞白血病的风险之间的优势比和95%的置信区间进行评估。结果 Meta分析结果显示母亲怀孕期间溶剂、油漆和石油暴露与儿童急性淋巴细胞白血病之间的关系差异有统计学意义,OR及95%CI分别为[1.25(1.09,1.45)],[1.23(1.02,1.47)],[1.42(1.10,1.84)]。而母亲怀孕期间吸烟与儿童急性淋巴细胞白血病之间的关系差异无统计学意义,OR及95%CI为[0.99(0.93,1.06)]。结论母亲怀孕期间接触溶剂、油漆和石油与儿童患急性淋巴细胞白血病之间的关系差异有统计学意义,尚不能得出母亲怀孕期间吸烟与儿童急性淋巴细胞白血病的关系差异有统计学意义。母亲怀孕期间避免环境和职业接触苯可降低儿童急性淋巴细胞白血病的发病风险。
Objective To evaluate the association between maternal exposure to solvent,paint,petroleum and smoking during pregnancy and risk of childhood ALL. Methods Relevant studies were identified by searching the Pub Med,EMBASE,Cochrane library and Web of Science databases. Odds ratio（ OR） and 95% confidence interval（ 95% CI） between the maternal exposure to benzene during pregnancy and risk for childhood acute lymphoblastic leukemia were assessed. Results Pooled odds ratio（ OR） with95% confidence interval（ CI） for ALL was[1. 25（ 1. 09,1. 45） ]for solvent [1. 23（ 1. 02,1. 47） ]for paint [1. 42（ 1. 10,1. 84） ]for petroleum exposure and[0. 99（ 0. 93,1. 06） ] for maternal smoking during pregnancy. No publication bias was found in this metaanalysis and consistent results were observed for subgroup and sensitivity analysis. Conclusion Childhood ALL was associated with maternal exposure to solvent,paint,petroleum during pregnancy in analysis of all studies combined and in several subgroups.Association with maternal smoking during pregnancy was weaker and less consistent. Avoidance of maternal occupational and environmental benzene exposure during pregnancy might contribute to the decrease in the risk of childhood ALL.
