摘要
目的研究调强放疗联合同步替吉奥化疗对直肠癌不良反应及艾迪注射液的干预效果。方法选择72例接受调强放疗联合同步替吉奥化疗的局部晚期直肠癌患者为研究对象,采用随机数字表法分为观察组和对照组各36例,观察组患者接受艾迪注射液治疗联合常规干预,对照组患者接受常规干预,比较两组患者近期疗效、不良反应、免疫功能、体力状况、生活质量。结果近期疗效：观察组有效率72.22%明显高于对照组47.22%（χ2=4.677,P〈0.05）;不良反应：观察组发生胃肠道反应、肝功能损伤、肾功能损伤、骨髓抑制、周围神经毒性等不良反应明显低于对照组（χ2=3.956~6.400,P〈0.05）;免疫功能：观察组患者血清白细胞介素-2（IL-2）、干扰素（IFN-γ）、肿瘤坏死因子-α（TNF-α）含量明显高于对照组,白细胞介素-4（IL-4）、白细胞介素-6（IL-6）、白细胞介素-10（IL-10）含量明显低于对照组（t=5.435~14.344,P〈0.05）;体力状况和生活质量：观察组Kamofsky评分、QOL评分明显高于对照组（t=3.011~6.620,P〈0.05）。结论调强放疗联合同步替吉奥化疗会造成胃肠道反应、肝功能损伤、肾功能损伤、骨髓抑制、周围神经毒性等不良反应,艾迪注射液有助于改善免疫功能,减少不良反应,提高生活质量。
Objective To study the adverse reactions of intensity modulated radiation therapy combined with concurrent S-1 chemotherapy on colorectal cancer and intervention effect of Brucea oil emulsion. Method 72 cases of rectal cancer patients accepted IMRT with concurrent chemotherapy were studied and randomly divided into observation group and control group,the observation group patients received Brucea oil emulsion combined with conventional intervention,the control group receiving conventional interventions. Then recent curative effect,adverse reactions,immune function,physical condition and quality of life were compared between two groups. Result Recent curative effect： observation group effective rate 72. 22% was significantly higher than the control group 47. 22%（ χ2= 4. 677,P〈 0. 05）; Adverse reactions： observation group gastrointestinal reactions,liver dysfunction,renal damage,bone marrow suppression,peripheral neurotoxicity were significantly lower than the control group（ χ2= 3. 956 ~ 6. 400,P〈 0. 05）; Immune function： observation group IL-2,IFN-γ,TNF-α content were significantly higher than the control group,IL-4,IL-6,IL-10 content were significantly lower than the control group（ t = 5. 435 ~ 14. 344,P 〈0. 05）;Physical condition and quality of life： observation group Kamofsky score,QOL score were significantly higher than that of control group（ t = 3. 011 ~ 6. 620,P 〈0. 05）. Conclusion IMRT with concurrent chemotherapy can cause gastrointestinal reactions,liver damage,kidney damage,bone marrow suppression,peripheral neurotoxicity and other adverse reactions,Brucea oil emulsion can reduce adverse reactions,improve immune function and quality of life.
