摘要
目的：研究老年结肠癌合并肠梗阻的诊断及手术治疗方法并分析其效果。方法：收集我院2012年2月-2015年2月间经纤维结肠镜检、x线钡剂灌肠、气钡双重造影、超声及CT等方式确诊为结肠癌伴肠梗阻的60岁以上患者共62例，对其临床资料进行回顾分析。结果：老年结肠癌合并肠梗阻的62例患者中，急诊手术者27例，择期手术者35例；左半结肠癌患者41例，右半结肠癌患者21例。其中急诊手术术后有8例患者发生并发症，并发症发生率为29．63％，择期手术术后有2例患者发生并发症，并发症的发生率为5．71％，择期手术的术后并发症发生率明显低于急诊手术，P〈0．05，差异具有统计学意义。行I期肿瘤切除及肠吻合手术的患者中左半结肠癌患者30例，右半结肠癌患者18例，共48例，左半结肠癌患者术后并发症的发生率为22％，右半结肠癌患者术后并发症的发生率为4．7％。结论：老年结肠癌合并肠梗阻患者需在诊断明确的前提下根据患者的具体情况合理安排手术，术后需加强护理，密切监测患者的情况，可有效预防并发症的发生，降低手术对机体造成的损伤。
OBJECTIVE ： To study the diagnosis and surgical treatment of colon cancer combined with intestinal obstruction in the elderly patients and to analyze its effect. METHODS： 62 elderly patients with colon cancer combined with intestinal obstruction from February 2012 to February 2015 in our hospital were selected and their clinical data was retrospectively analyzed. RESULTS： Among them 27 patients were treated by emergency surgery and 35 patients by elective surgery. 41 patients had left colon cancer and 21 right colon cancer. Among the patients treated by emergency surgery, 8 patients had postoperative complications and the complication rate was 29. 63%. In the patients cured by elective surgery, 2 patients had postoperative complications and the incidence of complication was 5.71%. The incidence of postoperative complications in elective surgery was siginificanly lower than that in emergency surgery. There was statistically significant difference （P 〈 0. 05）. Among the patients treated by one - stage tumor resection and intestinal anastomotic stoma surgery, the patients with lefe colon cancer were 30 and the ones with right colon cancer were 18. The postoperative complication rate was 22% in the patients with left colon cancer and was 4. 7 in the ones with right colon cancer. CONCLUSIONS： Colon cancer combineed with intestinal obstruction in the elderly patients should be clearly defined in accordance with the specific cir- cumstances of these patients in order to arrange the surgery reasonably. Postoperative strengthen care and close monitoring are needed. All of them above can prevent the complications and reduce the damages caused by surgery effectively.
