摘要
随着CT技术的日臻发展和完善,冠状动脉CT血管成像(CCTA)在冠心病的诊断中地位越来越突显,这种技术主要体现在不断提高的时间和空间分辨力上。64层甚至更高的MDCT可获得高精准的诊断率,同时也被广泛认为是无创冠状动脉血管检查的可替代方法。近来临床试验和科研通过CCTA更多倾向于对冠状动脉斑块的形态特征以及患者预后相关性的研究。本文简要讨论冠状动脉CT成像技术发展过程、斑块形态描述及界定与SPECT核素心肌功能显像比较。希望在临床检查中能够有全面、多模态影像检查方法作为无创检查金标准来界定斑块形态以及患者预后。
Coronary CT angiography has been increasingly used in the diagnosis of coronary artery disease owing to rapid technological developments, which are reflected in the improved spatial and temporal resolution of the images. High diagnostic accuracy has been achieved with muhidetector CT scanners (64 slice and higher), and in selected pa- tients coronary CT angiography is regarded as a reliable alternative to invasive coronary angiography. Recently, clinical practice and research are increasingly shifting toward defining the clinical implication of plaque morphology and pa- tients outcomes by coronary CTA. In this review we discuss the development of technical aspects of coronary CTA and the coronary plaque morphology estimated by CTA beyond coronary angiography including the comparison to Stress nuclear MPI using single-photon emission tomography. It hopes that an integrated, muhi-modality imaging approach will become the gold standard for noninvasive evaluation of coronary plaque morphology and outcome data in clinical practice.
