摘要
目的 ：建立老年糖尿病足溃疡＂医院-社区-家庭＂延续护理模式并探讨其应用价值。方法 ：2012年12月—2014年12月对华东医院收治的24例老年糖尿病足溃疡患者进行延续护理,观察患者足溃疡的转归。结果 ：22例老年糖尿病足溃疡患者创面愈合,占91.7%,愈合时间为36-96 d,平均72 d。2例患者因病情变化再次入院,溃疡未愈合。结论 ：＂医院-社区-家庭＂延续护理模式能有效促进老年糖尿病足溃疡的愈合。
Objective： To establish the extended nursing mode of ＂hospital-community-family＂ for the elderly diabetic foot ulcers and assess the application value. Methods： Twenty-four cases of the elderly diabetic foot ulcers who were admitted into Huadong Hospital for treatment from December 2012 to December 2014 were given the extended nursing to observe the outcome of the foot ulcers. Results： The foot ulcers of twenty-two patients were healed, accounting for 91.7%, and the healing time ranged from 36 to 96 days, with an average of 72 days. Two patients were re-admitted into the hospital because of the changes of their illness and their ulcers were not healed. Conclusion： The extended nursing mode of ＂hospital-community-home＂ can effectively promote the healing of diabetic foot ulcers in the elderly diabetic patients.
