管路封堵器在输尿管镜钬激光碎石中的适用性探讨 被引量:4

摘要 目的探讨管路封堵器在输尿管镜钬激光碎石中的适用性。方法选择136例单侧输尿管结石患者,其中研究组(68例)采用输尿管镜下钬激光碎石术联合管路封堵器,对照组(68例)仅采用输尿管镜下钬激光碎石术。分别比较两组输尿管中上段结石和输尿管下段结石的手术时间、输尿管损伤比率、结石上移比率、术后发热比率和结石清除率。结果在输尿管中上段结石患者中,研究组手术时间显著低于对照组,结石上移、术后发热的比率显著低于对照组,结石清除率显著高于对照组(P均<0.05)。在输尿管下段结石患者中,研究组结石上移发生率显著低于对照组(P<0.05),其余各项在两组之间无统计学差异(P>0.05)。结论输尿管镜钬激光碎石联合管路封堵器治疗输尿管结石具有结石上移发生率低、结石清除率高、安全有效的特点,值得临床推广。 Objective To explore the applicability of pipeline closure device in ureteroscopic holmium laser lithotripsy. Methods A total of 136 cases of unilateral ureteral calculi were allocated in this study,they were divided into study group( n = 68 cases) by using holmium laser lithotripsy under ureteroscopy combined with pipeline closure device and patients in control group were only applied with holmium laser lithotripsy under ureteroscopy. The operating time for segmental ureteral calculi and lower ureteral calculi,rate of ureteral injury,transfer rate of stones,postoperative fever and rate of stone clearance between these two groups were compared. Results In patients with upper section ureteral calculi in study group,the operating time was significantly lower than that of control group( P〈0. 05). The rate of stone moved upward and proportion of postoperative fever were significantly lower than those of control group,and the rate of stone clearance was significantly higher( P〈0. 05). In patients with ureteral stones in lower section,the incidence of calculus moved in study group was significantly lower than that of control group,but the rest had no significant difference between these two groups( P〈0. 05). Conclusion Ureteroscopic holmium laser lithotripsy combined with pipeline closure device in treatment of ureteral calculi have lower rate of occurrence of migration of stones and higher rate of stone clearance,hence it is a safe and effective procedure and it is worthy to be promoted.
