摘要
[目的]观察清热化痰汤治疗痰热郁肺型小儿肺炎支原体感染支气管炎疗效。[方法]使用随机平行对照方法,将86例门诊患者按抛硬币法简单随机分为两组。对照组43例阿奇霉素,第1d剂量为10mg·kg^(-1),第2~7d剂量为5mg·kg-1,1次/d,饭前1h口服。治疗组43例清热化痰汤(麻黄蜜炙3g,苦杏仁6g,生石膏先煎10g,瓜蒌仁、百部蜜炙各6g,川黄芩3g,川贝研末冲服1.5g,桔梗、板蓝根各6g,甘草3g;体温较高加鱼腥草、金银花;咳嗽痰多加青黛、紫苑;食欲不振加陈皮、山楂;大便秘结加大黄、火麻仁;喘息甚加白果、葶苈子),1剂/d,武火煮沸后文火熬制,水煎400m L,早晚餐后30min口服,200m L/次。连续治疗7d为1疗程。观测临床症状、哮鸣音、ELISA、不良反应。治疗1疗程,判定疗效。[结果]治疗组痊愈10例,显效15例,有效18例,无效0例,总有效率100.00%。对照组痊愈8例,显效14例,有效16例,无效5例,总有效率88.37%。治疗组疗效优于对照组(P<0.05)。症状消失时间治疗组改善优于对照组(P<0.05)。[结论]清热化痰汤治疗痰热郁肺型小儿肺炎支原体感染支气管炎疗效满意,无严重不良反应,值得推广。
[Objective]To observe the phlegm phlegm Yu Treated Mycoplasma pneumoniae infection in children with pulmonary bronchitis.[Methods]Randomized parallel controlled method,86 outpatients by disease Zhi No.lottery method is simple and randomly divided into two groups.The control group of 43 patients with azithromycin,the first 1d dose of 10mg·kg-1,the first 2~7d dose of 5mg·kg-1,1 times/d,orally 1h before meals.The treatment group 43 cases Qingre Huatan Tang（Mahuang 3g,Kuxingren 6g,Shengshigao 10 g,Gualouren,Baibu each 6g,Chuanhuangqin 3g,Chuanbei 1.5g,Jiegeng,Banlangen each 6g,Gancao 3g;high temperature plus Yuxin gcao,Jinyinhua;cough,phlegm plus Qingdai,Ziyuan;loss of appetite add Chenpi,Shanzha;constipation increased Dahuang,Huomaren;respite even add Baiguo,Tinglizi）,1 agent/d,heat to boil simmer after boil,cook 400 m L,morning and evening meal 30 min oral,200 m L/times.Western medicine with the control group.7d for a course of continuous treatment.Observation of clinical symptoms,cough,wheeze,adverse reactions.One course of treatment,to determine efficacy.[Results]The cure 10 cases,15 cases markedly effective in 18 cases,ineffective 0 cases,the total efficiency of 100.00%.The control group recovered eight cases,14 cases markedly effective in 16 cases,5 cases,the total efficiency of 88.37%.Treatment group than the control group（P0.05）.Time treatment group improved symptoms than the control group（P0.05）.[Conclusion]Qingre Huatan Tang Treated pulmonary bronchitis in children with Mycoplasma pneumoniae infection results were satisfactory,no serious adverse reactions,it is worth promoting.
