摘要
目的探讨虚拟现实术前计划辅助后颅窝脑膜瘤切除术的临床疗效。方法 13例后颅窝脑膜瘤患者分别行脑血管CT和头颅MRI检查。影像数据输入虚拟现实系统进行三维重建和术前计划,判断神经血管和肿瘤的空间关系指导手术,术后进行合并症以及随访疗效判定。结果虚拟现实术前计划中后颅窝脑膜瘤与周围神经血管的显示清晰。脑膜瘤切除Simpson分级Ⅰ级5例(38.46%),Ⅱ级5例(38.46%),Ⅲ级3例(23.08%),术后无一例出现严重合并症,出院时KPS评分均在90分以上,随访期间无一例复发。结论虚拟现实术前计划用于治疗后颅窝脑膜瘤有助于清晰直观地显示肿瘤与神经血管的空间关系,确保手术安全和有效。
Objective To evaluate the curative effect of microsurgery assisted by virtual reality preoperative plan for meningioma in posterior fossa. Methods CT angiography and head MRI were performed to 13 cases of meningioma in posterior fossa,and then,image data were input into Vitrea virtual reality system to perform three-dimensional reconstruction and preoperative plan. Spatial relationships among tumor,vessels and nerves were identified to direct operation. Postoperative complication and follow-up curative effect were observed and recorded. Results Visualization effects of spatial relationship among nerves,vessels and meningioma in the posterior fossa were well. Simpson grades of tumor resection extent were Ⅰ in 5 cases( 38. 46%),Ⅱ in 5 cases( 38. 46%),and Ⅲ in3 cases( 23. 08%). There were not cases with severe complications. KPS scores for all cases were more than 90 at discharge. Recurrence of tumor did not take place during follow-up. Conclusion Virtual reality preoperative plan is helpful for displaying spatial relationship among tumor,vessels and nerves clearly and objectively,which ensures secure and effective results.
