摘要
目的:探讨处方点评在提高处方质量、规范医疗行为和提高合理用药水平中的作用。方法:随机抽取南充市中心医院(以下简称"我院")2012—2014年门、急诊处方各67 566、76 833、86 579张,住院医嘱单每年各600份,采用临床药学管理系统软件及专业人员人工点评相结合的方法,对所其进行逐一审查,将所得数据进行整理分析及评价,并通过用药教育及行政干预等措施对用药不合理现象进行整改。结果:我院2012、2013年门诊处方基本指标的突出问题表现为抗菌药物使用比例、注射剂使用比例偏高,基本药物使用率较低,患者关怀指标不理想;不规范处方表现为适应证不适宜、遴选药品不适宜、给药途径不适宜及用法、用量不适宜等。通过用药教育及行政干预等措施,各项指标合格率逐年上升,3年的处方合格率分别达到了89.3%、96.3%、98.7%。结论:通过实施处方点评、用药教育、行政干预等措施,可显著提高处方合格率,使不合理用药现象得到持续改进,规范医疗行为,确保医疗安全。
OBJECTIVE：To probe into the effects of prescription reviews in the improvement of prescription quality,standardization of medical practices and improvement of the levels of rational drug use.METHODS：67 566,76 833 and 86 579 prescriptions in the outpatient and emergency department and annually 600 inpatient medical orders during 2012-2014 in Nanchong Central Hospital（ hereinafter referred to as ＂our hospital＂） were randomly selected.The clinical pharmacy management system software combined with professional prescription reviews were adopted to examine these prescriptions and medical orders.The gained data would be statistically summarized and analyzed.The irrational phenomenon of drug use would be rectified through medication education and administrative intervention measures,etc.RESULTS：The prominent problems of the basic indicators in outpatient department of our hospital in2012 and 2013 were the high application proportion of antibiotics and injections,the low application proportion of essential drugs and the unsatisfactory of patient care indicators.Non-standard prescriptions were inappropriate indications,improper selection of drugs,irrational route of administration and incorrect dosage and usage.Through medication education and administrative intervention measures,the qualified rate of indicators had been increasing yearly,which had respectively achieved 89.3%,96.3% and 98.7% during the three years.CONCLUSIONS：By prescription reviews,medication education,administrative intervention measures,the qualified rate of prescription can be significantly improved,the phenomenon of irrational drug use can be continuously improved and medical practices can be standardized,also,medical safety can be ensured.
