摘要
目的通过超声监测妊娠期糖尿病(GDM)胎儿脐动脉(UA)、大脑中动脉(MCA)血流参数,探讨其在胎儿宫内缺氧中的诊断价值。方法研究组为妊娠期糖尿病单胎孕妇92例,对照组为正常妊娠单胎孕妇172例,应用超声测量UA、MCA血流参数:收缩期峰值流速/舒张末期流速(S/D)、搏动指数(PI)、阻力指数(RI),计算MCA-PI/UA-PI值,研究组及对照组按照不同妊娠结局分别分成正常妊娠结局及异常妊娠结局两类。结果正常妊娠结局胎儿中,研究组MCA-S/D值及UA-S/D值、MCA-PI/UA-PI与对照组间有明显差异(P<0.05),MCA-PI/UA-PI均>1;研究组中不良妊娠结局胎儿MCA、UA血流参数异常的例数明显高于对照组,MCA-PI/UA-PI<1的例数也明显增多。结论通过超声监测妊娠期糖尿病胎儿脐动脉、大脑中动脉血流参数,能够预测胎儿宫内缺氧及不良妊娠结局,为临床及时诊治提供可靠依据。
Objective To evaluate the diagnostic value of the gestational diabetes fetal umbilical artery and middle cerebral artery blood flow parameters for the gestational diabetes fetal for fetal hypoxia. Methods Ninety-two cases of pregnant women with gestational diabetes fetal were in research group. One hundred seventy- two normal pregnant women were in control group. The resistance index (RI) ,pulsatility index (PI)and S/D ratio of fetal UA and MCA were detected by color Doppler ultrasound , and MCA-PI/UA-PI were calculated. Two group were divided into two subgroup:well outcomews and adverse outcomes. Results Compared with well outcomes fetal, the S/D ratio of MCA and UA and MCA-PI/UA-PI in two group was significant difference(P 〈0. 05). The cases of adverse outcomes were increased in group of GDM, and MCA-PI/UA-PI 〈 1 in more cases in group of GDM. Conclusions Through the detection of GDM fetal UA and MCA blood flow spectrum, we can predict fetal distress and adverse pregnancy outcomes for clinical treatment and prognosis.
