呼吸机相关性肺炎患者病原菌分布与C-反应蛋白及降钙素原水平的影响研究 被引量:27

摘要 目的研究呼吸机相关性肺炎患者病原菌分布及C-反应蛋白(CRP)和降钙素原(PCT)水平的影响,为临床诊断与治疗提供参考。方法选择医院2011年2月-2014年5月诊治的170例呼吸机相关性肺炎患者为感染组,以同期使用呼吸机未感染患者110例为未感染组,健康体检者80名为对照组,分析感染患者的病原菌分布,采用酶联免疫吸附测定法检测患者感染病原菌后血清CRP和PCT及炎症因子的水平变化。结果 170例呼吸机相关性肺炎患者中分离病原菌187株,其中革兰阴性菌77株占41.18%,革兰阳性菌110株占58.82%;呼吸机相关性肺炎患者血清中PCT、CRP及炎症因子肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)及白细胞介素10(IL-10)水平明显高于对照组和未感染组,差异有统计学意义(P<0.01)。结论呼吸机相关性肺炎患者感染病原菌以革兰阳性菌为主,且血清中PCT和CRP水平明显上升。 OBJECTIVE To explore the pathogen distribution and its influence on C reactive protein(CRP)and procalcitonin(PCT)in pneumonia patients induced by ventilation so as to provide reference for clinical diagnosis and treatment.METHODS Totally 170 cases of patients with ventilation-related pneumonia in our hospital from Feb.2011 to May 2014 were enrolled as the infection group in this research.Meanwhile,110 uninfected patients using ventilation during the same period were set as the non-infection group and 80 healthy individuals undergoing physical examinations were set as the control group.The pathogen distribution was assayed and serum CRP and PCT were detected by ELISA.RESULTS Totally 187 strains of pathogens were isolated from the 170 patients,including77(41.18%)gram positive strains and 110(58.82%)gram negative strains.The serum PCT,CRP,IL-6,IL-10 and TNF-αconcentrations were increased greatly when compared with controls(P〈0.05).CONCLUSIONPathogens isolated from pneumonia patients mainly consist of gram positive ones and serum PCT and CRP were increased greatly in pneumonia patients induced by ventilation.
