摘要
目的:分析幽门螺杆菌细胞毒素相关蛋白(cagA)、空泡毒素相关蛋白(vacA)基因分型在2型糖尿病(T2DM)患者中的表达,探讨其对T2DM的影响. 方法:选择2012年3月~2014年8月来我院就诊的2型糖尿病患者100例作为研究对象,设为T2DM组,选择同时间来院就诊的非糖尿病患者100例作为对照组,分别采用血清抗 Hp-IgG 检测、14C尿素酶呼气实验和快速尿素酶试验进行 Hp感染的确定,采用测序的方法对Hp进行基因分型,筛查Hp感染的cagA、vacA基因分型. 结果:T2DM组中HP感染者63例,占63%,未感染者37例,占37%;对照组感染者27例,占27%;糖尿病组患者感染HP和未感染HP的血糖差异无同学意义( P>0.05),但均明显高于对照组(P<0.05);高密度血脂比较,对照组明显高于糖尿病组,差异有统计学意义(P<0 .05 ) ,糖尿病组中感染HP和未感染HP者差异无统计学意义,低密度血脂糖尿病组感染HP者明显高于对照组和未感染HP糖尿病者,差异有统计学意义(P<0.05),对照组感染HP者中cagA占比为27.03%,T2DM组感染HP者中占比为:76.19%,T2DM组感染HP者明显高于对照组患者(P<0.05),对照组感染HP者中vacA占比为45.95%,T2DM组感染HP者中vacA占比为76. 47%,T2DM组感染HP者明显高于对照组患者(P<0.05). 结论:2型糖尿病患者易感染幽门螺杆菌,且HP的两个毒力基因ca-gA、vacA的存在会加重T2DM的病情并会出现后期的胃炎等并发症。
Objective:To analyze the Helicobacter pylori cytotoxin associated gene (cagA), vacuolar associated protein toxin (vacA) genotyping in type 2 diabetes mellitus (T2DM) patients express, explore its impact on T2DM.Methods:March 2012 -2014 in August to our hospital 100 patients with type 2 diabetes as an object of study , to T2DM group, select the same time to the hospital for treatment of non-diabetic patients with 100 cases as the control group, respectively serum anti -Hp-IgG detection, 14C urease breath test and rapid urease tests Hp infection were determined using sequencing methods for Hp genotyping screening Hp infection cagA , vacA genotyping . Results:T2DM group HP infected group accounted for 63 cases, accounting (63%), uninfected group accounted for 37 cases, account-ing (37%);infection control group accounted for 27 cases (27%);, diabetes group patients infected with HP and HP glycemic differ-ences uninfected students no significance (P〉0.05), but were significantly higher (P 〈0.05);high-density lipids compared to the control group was significantly higher than that of the diabetic group , the difference was statistically significant (P 〈0.05), diabetes group infected and uninfected HP HP were no significant differences , low-density lipids in diabetic group was significantly higher than those infected with HP and HP diabetes uninfected , the difference was statistically significant (P 〈0.05) in the control group who cagA HP infection accounted for 27.03%, T2DM group of persons infected with HP accounted for:76.19%, T2DM group were significantly higher in patients with HP infection (P 〈0.05), HP infection control group vacA who accounted for 45.95%, T2DM group of infected persons vacA HP accounted for 76.47%, T2DM group were significantly higher in patients with HP infection (P 〈0.05).Conclusion:Patients with type 2 diabetes susceptibility of H .pylori, and the two virulence genes cagA HP's, vacA presence of the disease would in-creas
