小剂量右旋美托咪定对罗哌卡因蛛网膜下腔阻滞效应及血液动力学的影响 被引量:11

摘要 目的探究小剂量右旋美托咪定对罗哌卡因蛛网膜下腔阻滞效应及血液动力学的影响。方法选择下肢骨科手术治疗患者52例,均以蛛网膜下腔阻滞方式进行麻醉。随机分成观察组(n=26)与对照组(n=26),观察组麻醉药物为3μg右旋美托咪定+5g/L罗哌卡因,对照组麻醉药物为5g/L罗哌卡因。观察2组麻醉效果、不良反应及血液动力学指标变化。结果观察组感觉阻滞起效时间、运动阻滞起效时间明显短于对照组,感觉阻滞时间、运动阻滞时间明显长于对照组,Bromage评分和舒适度评分显著优于对照组(P<0.01)。2组给药0、10、30、60、90min时血流动力学指标差异无统计学意义(P>0.05)。2组不良反应发生率差异无统计学意义(P>0.05)。结论小剂量右旋美托咪定对罗哌卡因蛛网膜下腔阻滞效果更佳,不良反应更少,且对血液动力指标无显著影响,具有较高的临床应用价值。 Objective To study the effect and hemodynamics of a low dose dexmedetomidine on ropivacaine subarachnoid block.Methods Fifty-two patients with lower extremity surgery were treated by subarachnoid block anesthesia.The patients were randomly divided into observation group(n=26)and control group(n=26).The patients in observation group were used with narcotic drugs 3μg dextral medetomidine+5g/L ropivacaine,while those of control group were treated with ropivacaine 5g/L alone.The effect of anesthesia,adverse reactions and hemodynamic parameters were observed in two groups.Results The onset time of sensory block and the onset time of motor block in observation group were significantly shorter than those of control group,but,the time of sensory block and exercise time in observation group were significantly longer than those in control group(P〈0.01).The Bromage score and comfort score in observation group were significantly higher than those in control group(P〉0.05).Two groups showed no significant changes in hemodynamic parameters such as MAP,SV,and CO after being treated 0,10,30,60,90minutes(P〉0.05).There were no significant difference in the incidence of adverse reactions between two groups(P〉0.05).Conclusion Low dose dexmedetomidine midazolam has good effect in ropivacaine subarachnoid block.The drug shows high clinical application value less due to adverse reactions and no significant effect for blood dynamic indexes.
